Please arrive 15 minutes before your first appointment in order to complete the necessary paperwork and obtain your insurance information. You will need to bring identification, such as a driver's license or state ID card, as well as any applicable insurance card(s) and any co-pay and/or referral required by your insurance.
Both forms are required prior to the release of records. The fee for service for these items is likely not covered by your insurance and will not be submitted to the insurance carrier for payment. You are responsible for the payment of these fees. Payment is due at time of request. Once the full payment is received, please allow two to three weeks for completion of your request.

I have been seeing Dr Nagesh for a few years now. He is gentle and listens to you with genuine concern. He has helped me keep my troublesome feet in good shape with his care and attention. He is personable and professional. I have my 97 year old mother also going to him. I highly recommend him and wish he could take care of other health issues and not just my feet.

I stubbed my toe on the mattress frame on Monday night, I went through the day on Tuesday in extreme pain and decided to call Foot & Ankle Associates since they're close to where I work. They were able to get me in the same day. They took x-rays and confirmed I had a fracture. I was so happy they were able to get me in so quickly!

Doc Grady is an excellent podiatrist. For over the past 20 years he has provided outstanding services for both me and my family. He has a consistent admirable team of docs and students who could only one day practice as well as john grady. The administrative staff is pleasant with welcoming attitudes and very specific in carrying out doctors orders.
