This will allow us to get started with your VIP LASIK evaluation as soon as you arrive. For patients who have scheduled for a Cataract Evaluation, we ask that you complete the following Cataract Questionnaire below. For patients who have scheduled for a dry eye assessment, we ask that you complete the following Dry Eye Questionnaire below.

Dr. Robert R. McCulloch is a Diplomat of the American Board of Ophthalmology and the Senior Cataract Surgeon with Horizon Eye Specialist & Lasik Center, having performed over 10,000 Lasik, and refractive treatments, as well as 20,000 cataract or corneal transplant procedures. As a specialist in refractive surgery, he also serves as Medical Director for our Lasik Center.
Dr. Angela Kovacik is a fellowship-trained Neuro-Ophthalmologist and Cataract Surgeon. She works within Horizon as both a General Ophthalmologist and Cataract Surgeon. She sees patients for cataract evaluations, glaucoma care, annual eye exams and eye emergencies as well as double vision, vision loss, optic nerve swelling and visual field defects, among others.

Poor customer service today. I ran out of contacts and went in to order some more. I asked if they had a spare trial I can buy until my order comes in the event I lost my only pair. They told me they didn't know and that they were not affiliated with the other office. I asked if they can call and ask but they refused and made me call them instead. True customer service doesn't exist at this office. They don't care if I can't see! I hope I don't lose my contacts until next week..
