Sadly, this place was bad. Believe the bad reviews you read here, they are true. Carmel Mountain Rehab needs to make some real changes and possibly the state should come to make sure things get better.

Any problems you find with the facility will be dealt with them promising to "look into it", "fix it", "take care of it".... BUT.... that WILL NOT happen, even when advocating (MANY times) for your loved one.

Carmel Mt. will "promise" to do it later, "call maintenance", but they will NOT do anything. When you check back you will have to complain

Our 81 year old father was released from the hospital to Carmel Mt. Rehab at the end of December 2021. It was a terrible experience. The last incident that occured before we checked him out was when his hospital bed collapsed causing injury to his leg and neck. He was quarantined due to being covid positive and staff had the doors closed to his room, with no routine check ins. He was to push his call button for help should he be in need of anything and he pushed and pushed it and verbally called for help but no one came. This incident in itself makes me want to cry.

Would Not recommend this facility .

My 85 yr old mother had a stroke after a two week hospital stay she went to Carmel Mountain Rehab.
The staff seemed to be oblivious to her needs her vision ,mobility and cognitively capabilities are affected any understanding of that seemed to not be considered by the staff.

One morning she was helped to the bathroom after waiting far to long and was left in there she passed out in the bathroom someone finally came to her aid called for help they put her back in bed and never told me or made any kind of report and did not notify the doctor.

I was a week out from open heart surgery. This Hell Hole was recommended.
Ambulance took me from Scripps Cardio Center and delivered me to this site. Took me on a long, dark, depressing gurney ride to the back of the facility and transferred to a dingy bed against a cold back wall. The rails on the bed looked like a rodent had been gnawing on them (I took photos.).
My legs don't work; a composite of Military disability and spinal stenosis.

My immediate relative came to this facility as it was offered to them through Scripps as a step down rehabilitation center. My relative went into this facility with a Temporary PEG as to preserve the cell lining in her esophagus due to breathing tubes and NG tube from after her bypass surgery/stroke. Her stroke left her unable to eat and with some neurological issues, so the PEG was precautionary and not permanent. At first we were hopeful of her recovery as they assured us this place was one of the best facilities in San Diego.

My mother suffered verbal abuse, neglect and mistreatment at the hands of CNAs, charge nurses and a certain Physical Therapist. Mom had several injuries and was to rehabilitate and recuperate at this facility, Her pain meds were withheld and or given at random intervals often leading to sleepless nights a and very depressing outlook for recovery. Diet restrictions were not addressed and mom suffered dangerous blood glucose levels, and other medical conditions due to lack of care. Carmel Mountain Rehab is a death trap. If you have family you would like to get rid of, send them to this facility. My mother received two medication errors. The second was 6 days of heparin she wasn't supposed to receive. She ended up with a hematoma on her leg in which a whole unit of blood bled into. The RN at the facility did not know what a hematoma looked like. When I identified it and got ahold of the doctor he had the entire staff come to her bedside to learn what a hematoma looked like. She died the next day at the hospital.

Carmel mountain rehabilitation may not be the best or perfect skilled nursing facility but the facility administration down to the line staff always strive to serve every single patient in the best way they can. Staff always ensure safety and happiness of every single patient. Covid 19 is very challenging but all the staff in this facility left their love ones at home to work and serve the patient not for the pay check but because they care about the patient. Staffing is very challenging nowadays not only in this facility but everywhere-you go but they never stop to care and provide services.

DON'T PUT YOUR LOVED ONE IN THIS FACILITY
At this point I am very disappointed and will never allow my mother to be placed in this center again. I understand the Covid precautions but they are holding my. mother hostage. I have to make an appointment to visit my mother, they won't return my calls so I can make that appointment and she sits in a room, alone, isolated and helpless. I am her care provider and my absence is affecting her recovery.

Do NOT send your loved ones to this facility!
We had our Father admitted to this facility in January of 2000. Within 13 days, he was transferred via ambulance to a hospital with the following issues: a stage 4 and untreatable bedsore, dehydration, malnutrition, blood poisoning (not monitoring his blood thinners), bed sores on his heels, and SEPSIS!
Facts matter. Truth matters. And I fully plan on using the pictures I personally own to help wake up the world to what is happening to our elderly! Horrifying is the BEST term one could possibly use to describe what the world is about to see. Dad was

My family is currently involved in litigation with this facility and I will update this review as that moves forward.

A brief summary of what happened with my father. He went there for the last step before coming home after a stroke. He left there 2 weeks later via ambulance and arrived at Pomerado Hospital with pneumonia, sepsis, internal bleeding, dehydration, malnutrition, and a grapefruit-sized bed sore on his tailbone. The ER physicians said he would never recover from these injuries and they were right. He ended up passing away.

This is the worst place to bring a loved one. They don’t take care of their patients if you are out there trying to look for a good place for your love ones please please please don’t put your family members in this place I regret so much how my dad passed away in this place. This is how my dad lived here his last days, these people never took care of his doctor’s appointments, never feed him right and neglected the whole time. It was hard for me to bring my dad due to i live In live in Texas, there was no way to bring him with me due to his medical condition.

Pros:
Workers seem caring (but the system is a mess).
Window visits anytime. (In person visits had to be scheduled in advance for a max 30 min and only a limited number of people could per day so it would often be a week out before being able to schedule a visit, even if both the visitor and patient are vaccinated. Might be a covid restriction though.)

Cons:
Did not give all the prescribed medication, did not give medication on the correct schedule, did not follow prescribed diet. (These things should be needing to be fought for and repeatedly reminded about over many weeks!) He did not

Great facility and great staff! Courtyard is a great outdoor space for the residents to get fresh air. You're welcomed by all the staff upon entering. The facility is well kept compared to other facilities.

The staff at this facility was SO helpful when my Grandmother was discharged for end of life comfort care. They made certain that we had everything we would need for her at home care and gladly answered any follow up questions that I had when I called. Specifically, Connie the case manager, was a giant help. Thank you to all of the staff who helped ensure that my Grandmother's passing was peaceful and comfortable. You made such a difference to me and my family.

My partner was admitted to the Carmel Mountain Rehabilitation Facility following hip replacement surgery in March 2020. After 3 weeks of extremely poor medical attention by the nursing staff and doctor, Bruce asked to be discharged.
He arrived home with blood on his pants. When viewed, there was a dressing filled with blood. The wound from hip replacement surgery had opened and infected. No paperwork. From Carmel Mountain Rehab, indicated any infected wound.
His orthopedic surgeon viewed the infection and immediately did surgery to clean the pus filled wound. Bruce was tested for MRSA and diagnosed

Pathetic and disgusting. Arrangements had been made 2 days prior to my mom being released from the hospital to stay here short term. One hour before discharge they backed out claiming 'insurance didn't go thru approval.' We found out the next day that they had decided to give her bed to another person whose insurance paid more. As a result my mom's hospital stay was extended as we scrambled to find another accommodation.

Places like this don’t get acknowledged enough by the community, rather deemed “horrible.” What other reviewers who provide low stars, fail to previously acknowledge is the workers in Carmel Mountain or any other healthcare facility.
We review horribly not because it really was a terrible experience. Perhaps it is our way of coping to our inability to care for ourselves or because we have some guilt that we rather rely on others to care for our family members. And at any point we see something we dislike we point it out. Maybe instead of being critical about the service we failed to receive

All staff here are so caring and Physical Therapists really know their stuff and so creative. I loved them ❤️ all. They get results and make the work as fun as possible. Truly, they care about YOU. I was there for a month or more. It doesn't take long to see the staff as friends that have your best interest at heart.
As an add-on to my review, I was sad to see a remark that the positive remarks are not true or paid remarks. That is sooo untrue and insulting. I was a real patient and I was treated very well. However, I did see some patients there that were just difficult to deal with daily

My husband was admitted to this facility for rehabilitation from a laminectomy surgery, but he also has Parkinson's. When I realized he would have to have more care than just coming home because of Covid, I was so worried. April, in admissions, reassured me, and the process went smoothly. The therapists who treated him had him up and walking in his clothes the very next day. We live in a two-story home, and they made sure he was secure with going up and down the stairs. Our father was admitted to the facility in March 2020 to early July 2020. When he was first admitted, his care was completed under the acute care area of the facility. He was then transitioned to standard care for the final duration of his stay. While under the management of the standard/nursing area of the facility, my father succumbed to several injuries due to the neglect of the nursing staff. His injuries included a predominant and incomprehensible bedsore on his posterior, a rash that encompassed his entire body, and a severe regression in his speech. Very cleanly community; in a very beautiful part of the county.
Carmel Mountain has a beautiful courtyard, right in the center of the community. The courtyard is a great place to just relax, or even receive therapy - due to the space in which it accommodates.
Every single person that I interacted with went out of their way to acknowledge my being in the community. That is something that I never overlook. You know that the team looks forward to patient care needs.
Scripps doctors round there as well, which was a plus for the residents that I help out in the community.

Carmel Mountain Rehab is by far one of the BEST Skilled Nursing Rehab Centers in the San Diego counties. I am an outside vendor to many skilled Nursing Facilities and assist families with placement upon discharge. I have been working with CMR for several years and consider them my professional family. Always greeted with a smile by all staff members and they always take a minute to ask if I need anything. Very attentive and caring to their Patients, professional and courteous. When I have a skilled nursing care/rehab need for a loved one Carmel Mountain Rehab will be the first I call!

I dont usually write reviews but I felt that the service received at this facility were next to criminal. My 83 year old mother suffered a Traumatic Brain Injury (TBI) on Christmas night and after 3 days in the ICU was sectenced to this facility. The food was always late and cold. I needed to take hot food to my mother every morning and every night. Their "call" button often took more than 20 min and the woman next to my mother spent more than 30 on a toilet waiting for help...twice!!

When we checked my mother in, we were told that we would be "in the loop" communication

This place perforated my father's rectum with a "Rough and painful" enema that caused a fissure and a bleeding lesion in his colon. He lost so much blood they had to call an ambulance and rush him to the hospital where he had to receive 5 UNITS of whole blood, and multiple liters of D5 to keep from bleeding to death from the injury. This lead to multiple extremely invasive procedures, and an additional 7 day hospital stay and set him back dramatically on his recovery from his original injury.
