On behalf of our entire team, thank you for considering our nursing and rehabilitation center for you or your family member's care. Our newly remodeled facility features 155 skilled nursing beds and includes a large rehabilitation gym with state of the art equipment to help accelerate the recovery process of our residents.
Healthcare Centre of Fresno provides everything from intensive short-term rehabilitation therapy to long-term restorative care. Our nursing and rehabilitation Centre has a full-range of nursing care, therapy and social services support to patients and residents. Rehabilitation is an essential component for a successful recovery, if you or a loved one has had a serious illness or surgery.
Since I had the opportunity to come to Healthcare Center of Fresno, my health has truly improved. The personnel and the Administrators have displayed concern and kindness and are very helpful. Last year, I weighed 501lbs, now I am at 318lbs. My true healing started here! The CNAs and Nurses are, in my opinion, Outstanding!

I’ve been living here for awhile. I love my responsibility as the Resident Council President and I love the Activity Department and Social Service Department. Every time I have family visits Activity Dept. always help me with my family visits. I enjoy Bingo, their special events and all activities they provide. I would like to thank Healthcare Center for their support and help. They earned my 5 star review and thank you to all CNAS and Nurses too.

Do not recommended this place to anyone!! if you do you better be checking on family member my son needed to go to hospital. While there he complained he was in pain a whole day. left with his vomit by his bed for hours till I Called his cell phone and heard how sick he was feeling and insisted they transport to hospital or I would go over there and take him myself then they finally did something they ignored his plea he was in pain I did not take him back after he got released 3 days later from hospital. He was having intestinal problems I decided he was better home than there. Did did nothing

TERRIBLE FACILITY! If I could give 0 stars I would. My grandmother was there recovering from an invasive brain injury. She was there for a few months during COVID-19, so none of us were allowed to see her. When we called, the nurses station would never let us speak with her or let her know we called. When we brought her home, she had baseball sized bed sores that were revealing her flesh and were completely infected. This place is disgusting and should be shut down.

If I could give this place ZERO stars I would, my grandmother was horribly neglected here. This place is very unsanitary. PLEASE do not send your loved ones here. NO ONE should be treated like my wonderful grandmother was treated. I am so happy my family took her out of here and my heart breaks for the elderly still here (probably unwillingly) putting up with their abuse. DO NOT USE THEM!!!

It is unsafe and unhygienic; the staff is poorly trained, unmotivated, and uncaring; the equipment is both inadequate and, in many cases, broken; and the nutrition is far below standard in taste, presentation, and literal nutritional value.

My grandmother was at this facility for 30 days following neurological care at Clovis Community and was released with a stage 4 bedsore and double pneumonia. Do not send your loved ones here. These people are not trustworthy and do not care in the least. They did not tell my family about her bedsore when they picked her up from this facility. My family had to SOAK her feet and hands to get them clean! This place should be shut down!!!

I have a family friend who has been here for the last two weeks. Yesterday the social worker from the facility called me to inform me he would be discharged at 10am today. This morning i called at 9:45am to inform them i would be on my way to pick him up so they can get started on the process of discharging him. I got there at 9:51am front desk told me they will bring him downstairs to wait outside. At 10:30am i went back to front desk and was told they are bringing him down any minute. At 10:33am one of the staff members who is a coordinator approached me asking if i was waiting for someone. I

Staff is excellent especially the activities personal and CNAs they go far and beyond to make sure during this Covid 19 lockdown to keep everyone comfortable and safe. It's unfortunate that some don't understand how hard it is to maintain a perfect atmosphere setting with a state run budget Hats off to the staff

I wouldn't even give the facility a one star. My grandmother had to come here after invasive brain surgery during COVID. The employees are horrible to work with. When calling the nurses station they would either not let us talk to her or no one would answer. If we did get someone they would tell us we call too much. They never turned my grandmother so when she came home she had a stage four bed sore. Almost to the point of no hope. She was so filthy dirty that we had to soak her hands and feet in water to get the dirt and grime off of her. The facility took no responsibility for how she was

I wouldn't even rate this a 1 star,its a dirty place the pictures they post on their site arent even the same as pictures, every room is dirty the beds are hard dirty,the people who work there dont care about patients my senior citizen was there ,they starved him he had bed sores that they never cleaned I cant believe with the money they receive from each patient they'd have a cleaner facility and a more professional staff even their social workers are lazy and not professional at all. If you love your family member find them another place the neglect is unacceptable

Horrible care facility with negligent staff. On 1/27/19 I found one of their patients sleeping on the street 2 miles away from the facility. He had been on the streets since the previous day having escaped. When I called the number on his wristband, the staff put me on hold, were rude and falsely promised to send someone to pick him up. Never happened, I had to call 911 myself to get him the care he needed and he was driven "home" in the back of a police car. Disgraceful treatment of an elderly gentleman and a sad reflection on society's priorities. Shameful.

This place is such a disappointment! The nurses are so mean and heartless, they simply don't care. The bed didn't have rails, which would cause the patient to fall at night. The cord to call the nurse doesn't work. Nurses were aggressive with the patients and didn't come in to the room to check in on the patient. This is wrong and needs to be addressed. If you love your family please don't bring them here.

The staff is negligent. They shuttled their patient to the VA for an appointment and left him waiting outside under 100+ degree weather for more than 45 mins. It didn't seem that they were planning on picking him up until a family member called them about it. The week prior, they didn't bother checking on the same person when he was in extreme pain and throwing up everything he was taking in for 3 days until a family member came to visit and pointed out his condition. He was sent to the emergency room and was diagnosed with having Gallstones, not to mention was also dehydrated from throwing up

My grand mother was there for 5 years. Due to situations out of my control I could not rescue her from there. She was first on the 5 floor for a long time, maybe about three years. She fell so many times I lost count, they never called to tell me. I would just walk in a there she would be with a black eye that covered most of her face (three times) an once she had to get stiches in the back of her head. I was suspicious... . My mother stayed there for 5 months. They do the absolute minimum required. Poor facilities: 1 of the 2 elevators was not operational for most of my mom's stay, so I normally took the stairs. Code required to turn off alarm when going through door to stairwell, yet alarm box did not have legible numbers on the buttons. Her room had patched walls, yet the white patches were not painted during her stay and did not match the color of the walls. Healthcare Centre Of Fresno is such a friendly place. Very clean and very pretty. The staff is so attentive to all the needs of the residents. They treat each other as well as the families of the residents like family. They have activities for all and they have so much fun. The foods was like eating at a restaurant, it is so good!

My grandma came here for rehab after her surgery. She got out as soon as she could. The bed was unmade when she got to her room. The staff did not come when we pressed the button. The food was terrible. She only got attention once we made it clear we were planning to leave (after just one day). Very unprofessional.

This facility lacks the proper care it's residents need. We came to visit our Grandmother on Thanksgiving and she was eating her dinner while sitting in her own urine when we arrived! I had to make a nurse change her into clean clothes. Unfortunately we don't have the means to move her to a better facility, but this is not right. The staff should be ashamed of themselves. They deserve less than one star.
