Legend Oaks Healthcare and Rehabilitation of Fort Worth is a modern, state-of-the-art skilled nursing facility, located in the heart of Fort Worth, Texas. We are on the east side of the Crawford Farms neighborhood, about a mile east of I-35W and Texas Health Harris Methodist Hospital. My best friends mom was found unresponsive in her room about 1030-11 am. The aid said she had been like that since 7 am. They left her in that condition for over 3 hours. She is being rushed to the nearest hospital and I pray she survives. The medic who transported her told the family it would be in there best interest to make sure she does not return there. They have been there on many occasions for cardiac arrest and they have lied about the patients conditions.
They have lied about the multiple burses on her body as well as a puncture on her back, which they are blaming on her Kidney doctor

Our family really appreciates the staff at Legend Oaks in Ft. Worth. Our mother has received good medical care , med management, and PT. The staff works hard to motivate her to get out of bed and move her body. The staff that we interact with are helpful and friendly and my mom has her favorites! At the end of the day this is a short-term rehab and long-term care facility not a five star hotel. Our mother needs more care than we can provide at home and Legend Oaks is a perfect fit for her needs and close to family members to visit. Thank you.

I wouldn't feed the food served there to my dog, seriously. The nurses are over worked without support aids. Response times to call buttons are slow often 20-25min once 45min. The therapists are the only thing good about this place they are wonderful. My mother has been here two weeks and not once has her room or bathroom been mopped. It's like the majority of the help could careless about doing a good job.

This place has no Communication ethics. Never once did I speak with the social Worker after the initial admittance of my husband. She said she tried many times via phone and she may have but I was there almost ALL DAY EVERY DAY and when I asked to speak with her they would tell me I’ll give her a message you’d like to speak with her
My husband did not always know what day it was or what year it was to the extent that he thought we were living in the year 2000 or 2010 yet they allowed him to sign papers to deny continued home health and helpful devices when I was not there

The staff at Legend Oaks is truly caring and treat the residents with dignity and respect. Amanda McNealy is extremely dedicated to ensuring not only the physical comfort of the residents, but is also concerned for the physiological safety of those with dementia. She is truly a blessing and made sure my Mother was well cared for.

The nursing staff is very dis organized no sense of urgency. Give you the wrong pills or say they don't have it the pharmacy has t delivered even though I brought my own medicine and was given to me the same morning only to be told at night they did t have. Somebody new it seemed everyday. Bed sheets were dirty and only changed after constant badgering. Call button seemed to be ignored half the time. Food always cold.

I recently placed my mother here for rehab. I was very nervous because of Covid and all of the restrictions with visiting. I must say that the staff here really went above and beyond to keep me in the loop on my mom's care. She loved her therapists and said they really pushed her to get stronger. They called me a couple times a week to give me updates and even helped me FaceTime her when I wanted to see her. Definitely seems like a group of people that truly care.

My family had a wonderful experience here at Legand Oaks Healthcare. My mother was a patient after she fell, broke her ankle, had surgery, and needed some rehab following. The entire staff was so friendly and helpful, communication was great and we stayed well informed about my mother's treatment here. The facility itself was very warm and inviting and I could tell my mother was in great hands. Also, she was there pre-COVID so we got to see her everyday, anything I needed, someone was there and willing to help. I would recommend this place in a heart beat.

My mother has been there around three years. At first it was fine but they've gone through several Directors and everytime it gets worse. Now they hold back pain med 2 to 24 and even 48 hours and each time it is a different excuse.. And if you call and complain to a nurse then they get treated differently by some. We are in the process of putting my Mother somewhere else because it's just not worth the fight. So i would think twice before putting a loved obe there. Looks can be deceiving!
