Services
Skilled Nursing is care provided by a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in accordance with a physician's signed plan of treatment in the comfort and security of a patient's home. The RN makes the initial evaluation visit and establishes a nursing Plan of Care (POC) for the patient.
The Physical Therapist is a qualified professional who directs, supervises, evaluates and provides physical therapy services to client/patients in the home as prescribed by the attending Physician. At Home Care believes rehabilitation is important both before and after surgery and has developed a integrative care plan to prepare patients before they enter the hospital to ensure the most successful recovery.
The Speech Therapist/Pathologist is a qualified professional person who directs, supervises, evaluates and provides speech and language therapy services to patients in their home as prescribed by the attending Physician. Speech Therapy (ST) services are invaluable for stroke, Parkinson's and other physically debilitative disease patients.
The Medical Social Worker is a qualified professional person who provides social case work and prepares social histories for patients and family receiving Nursing or other services from the agency and who have problems in social functioning as it relates to their health and well-being. These workers are the link between the patient and his/her family and the community resources available including meal delivery, counseling and patient advocacy.
