I can’t emphasize enough on the amazing team work I’ve witnessed with this building from each department. It amazes me how much support they provided for one another because at the end of the day we are here for one reason and that’s to care for our patients/residents. The care provided by the CNAs and NURSEs are extraordinary and highly contagious to the supporting staff!

My client was there for 2 weeks, and when she came home, her pants were wet, and she had one of the most worst rashes I have ever seen in my life !!!! She went there 2 months ago, only for a week and came home with rash then too . This time though, the rash is from front to back, between her legs and very very red, with peeling skin. They never changed her and just let her sit in a wet diaper for days !!! How sad! If you don't have enough staff then don't accept so many clients. They should hire someone just to change diapers ! My clients husband tried to call but of course, no call back.

Run away!!!
This place did not provide good care on any level. Our two week experience included two ER visits with the last one resulting in hospital admission. Instead of getting better under their watch, my father is now much worse off than when he checked in.
If I could, I would give them less than one star.
Unless you want your loved one to suffer, avoid this place at all costs!

Do NOT allow your family members to be admitted to Citadel post-acute. They have started accepting COVID positive patients starting October 18th, 2021 from the hospital without protocols and procedures in place. My family member (as well as the whole 300 wing) became positive for covid after this patient arrived. The physician has ordered monoclonal antibodies which is standard of care at the beginning of the week. However the facility does not have the logistics to provide the monoclonal antibodies to the patients in which this treatment is very time-sensitive. I have called everyday and attempted

DO NOT send a loved one here. My dad was here for 14 days and on his 14th day he was positive for covid. Nobody returns your calls. I have called dozens of times in the 2 weeks and if the front desk even answers they say they will leave a note. I have guardianship over my dad due to his stroke and not once did they call me to discuss his care plan. They put him in a broken bed even though he is a high fall risk. They only gave him one shower even though I constantly left messages. His room phone was broken so it was extremely hard to reach him. One of the times he fell they sent him to the hospital

I am a former CNA of Citadel Post Acute. I enjoyed working at Citadel and hope to return soon. I had to leave for a short time to get some acute care experience, but I stayed on at Citadel as a volunteer for a short time because I loved and appreciated the staff and residents so much!
Marla, the DON, and Terry, the Administrator, do so much to care for both employees and the residents. I transferred here from another facility and I knew I had made the right choice when I saw Terry getting ready to leave for the day and saw a CNA stressed about getting her charting done because she couldn’t sign

Beautiful well-maintained community, and grounds, wonderful employees and top-notch resident care. You can tell the employees care about themselves and those they take care of. Especially Lakisha Love I was very impressed. I'd move my loved ones here, no doubt!!

If I could give a negative review, I would. This is a horrible facility and I would never recommend it to anyone, especially an elderly person who is confused, ill and in need of quality care. Let me start off by saying I understand policies have changed some due to COVID; however, I do not believe not involving the family in treatment plans of a patient is an acceptable practice in any situation. I could write a very lengthy review detailing every aspect of my dad’s care and the contact the family had with this facility. I’ve decided to be as brief as I can. The visiting policy is terrible

Hello there my mom was just discharged from Citadel Post Acute. She finally received the care and staff she deserved. This facility earned my mothers trust in helping her understand that she should not be living on her own and helped her get the physical therapy and occupational therapy she needs. With Covid going on the phone calls were slow coming in from staff however I know they were busy and they were taking care of their patients. My mother had been in a facility prior which was Alta Mesa which gave me the runaround that she was in a safe environment and she was an "unsafe discharge

Absolutely thee worst place to send your loved one! My grandmother spent time in this horrific facility and developed a awful bed sore that ended up sending her to ER & PCU care for sepsis/blood infection.

Several different physicians state this was due to staff not turning her regularly and could have def been prevented. LPN "Stephanie" demonstrates lousy customer service & patient care. This place is a disgrace an would NEVER recommend anyone enter this facility. We are looking into further investigation && seeking legal action so this situation doesn't occur

Where do I begin? My mother has been in this facility for a few weeks now. Even though the staffing ratios were poor, she waited for hours for meds, and the food was unrecognizable we kept her there because while on PT she was getting better. While waiting for her ALTCS application to be approved, the nurse case manager had my mother sign a paper to agree not to receive anymore PT since her Medicare benefits had exhausted. My relative was in there for 2 weeks he was not shaven and his appetite was down and the nurse stated that he did not "feel good", so this is why he was not eating!

I would recommend not using this facility with having to have NPs, CNAs, nurses give patients healthy grooming everyday, have them eat even though they do not say they want to this is what they are getting paid to do be a care giver!

We should have been FaceTiming every day to see what was really being done or NOT Done due to COVID-19 you are not let in the facility and within one visit you would access the situation on

If I could I would give zero stars. My mother was in hospice dying I walked outside to get some air and warm up in the 84° sun when I walked back in I had a slight increase in temperature from standing in the sun to warm up. I have been in and out of the front door five or six times today. My temperature never got above 98°. Standing in the sun to warm up with a jacket on it reached 100. I was forbidden to ever return to the facility as my mother lies in bed dying alone. Not to also mention my daughter was standing by the reception when April and Jesus said I don't care if her mother is

Nice facility! I came as a guest to a family. The entrance in the lobby was very inviting, and appropriately decorated for the season. The nurses station was neat and open. I went to speak with a staff member there and Mark was more than willing to help me, twice in fact. I noticed that the meal that was served to the patient, looked tasty and appealing, and something that I would be willing to eat myself, complemented with a dessert. Again, this is my initial impression and having been here this being my second time, seems like a friendly, clean and a nice atmosphere in which to do rehab at

My fiancé was transferred to Citadel after a hospital stay. The place has not been updated since the 70's. It is not inviting and reeks of urine. The staff are not helpful. My fiancé had to constantly remind them of his gluten and dairy allergy. They would say they would check on it and not come back. I'm trying to get him home or transferred to a new facility.

My dad spent a week at Citadel Post Acute recovering from heart surgery. The staff was positive and extremely attentive to my dad's care. They paid attention to small details that made him feel more comfortable, such as a second cup of coffee. The therapists worked with him and kept his program upbeat and positive. My dad is home now but stated that he had excellent care at Citadel.

As the family member, I was kept informed of my dad's progress. The case manager, head nurse, and the entire team kept me up to date and addressed my concerns. When my dad was discharged, i felt confident

My husband stayed at Citadel for 2 weeks after a being transferred from another facility. The CNAs were really great and caring. He was completely bedridden and had to rely on them for everything, including brief changes. They were always prompt and caring. Shout outs to Laura, Carly, and Alexis but they were all great. Also our case manager Jackie was awesome and and so incredibly helpful. I would have truly been lost without her direction. The only reasons I don't give it 5 stars is because they are not equipped to deal with mental health issues, (in my opinion). My husband is bipolar

For my school, we had to come here for our clinicals for our program. The actual CNAs (certified nurse assistants) were so rude to all the residents. They would force feed those who needed to be fed, the way that they talk to the residents were so rude, and everything. One of the residents said she got a bruise from the Hoyer lift (to help the residents to get into their wheelchair and back to bed when needed) because she has been hit by two of the CNAs and those CNAs were not even sorry. As students, we were not allowed to be left alone with the residents, unless we were just having normal conversations

Absolutely do NOT recommend! My dad was transferred from the hospital to Citadel after he had a mini stoke (and back-related left leg weakness/pain).

He was there LESS THAN 12 hours (probably about 8.5 hrs, so 1am) and he needed help to the restroom. He pushed the call button for the Nurse several times and NOBODY came. The (shared) room was tiny and his bed was probably 10-15 steps from his bed. He was tired of waiting and decided to make the trip himself however, I think almost as soon as he stood from the bed, he fell and hit his head!

I found that the help was rude. they were slow to respond. they seemed to argue with each other and complain out loud in front of patients and families. trying to get through to anyone on the phone was a joke. waited mulitple times almost 30 minutes and finally hung up. asked for call back from nurses. never received calls back. wouldn't recommend this place!
