Services
A physician's prescription for therapy is required if we are to help bill your insurance carrier as a courtesy to you. FAX the order to (602) 249-4115. As Arizona is a direct access state for our PT and OT licenses we are able to see you for any service without the prescription of a physician, on a cash-only basis.
Located 1/4 mile south of Northern Avenue on 19th Avenue, our red brick two story medical office building "7540" is two buildings to the Southwest of the intersection of Morten Avenue and 19th Avenue. We are 1/8 mile north of Orangewood Avenue, and north of Glendale Avenue. Free covered parking is located on the west entrance to our building.
From personal notes to Patient Satisfaction Surveys to phone calls, we value the feedback. The courtesy of letting our team know of any concerns so we can immediately help is appreciated. The highest testimonial is informing the physician of appreciation of being referred to SWTS. About a year ago, I developed a very painful case of lateral epicondylitis, aka tennis elbow.
Surveys are also utilized as part of the overall quality assurance methods and we participate in these Focus on Therapeutic Outcomes (FOTO). At admission, each patient completes an assessment(s) specific to the body part or impairment that needs treatment. From these responses the functional status measure score is calculated.
Each session has one-on-one instruction by Southwest Therapy Specialists' caring, licensed Physical Therapy team. Physical Therapy (PT) is a form of health care that identifies, corrects, and alleviates an acute or prolonged musculoskeletal or neurological dysfunction. Our Physical Therapists have either a Master's or Doctorate Degree in Physical Therapy and are licensed in Arizona to practice.

Jim and Christine are great; sad they have to work with an owner that plays games and acts as if she is "always there for her patients"...something is wrong when your staff doesn't stay maybe it starts@ the head....water pt. 1day a week...Although offered the owner never gets in and they double and triple book people the same time...forget getting one on one..unless your private pay and wealthy than u have all the attention in the world...

Where to start...Susan, Jim, and their phenomenal staff have taken a journey with me. Susan's skill and expertise is owed to her outstanding thirst for knowledge and dedication to getting her patients the care they deserve and sorely need. When all other "Dr.s" wanted to give half-thought up excuses and sordid theories without even bothering to touch my wrist, Susan asked for precise descriptions.
