FORM Physical Therapy is a small independently owned boutique practice located in San Francisco's inner Richmond neighborhood, Sausalito, and in downtown Tiburon. We offer care without the use of aides or excessive modalities. Each visit is approximately 55 minutes and the focus is manual therapy of the affected muscles, joints, and/or nerves coupled with specific exercises to get you back to life and sport.
Services
I take Medicare/ Medicare Secondary, you are only responsible for any deductible or copayment amounts. All other insurances are considered out of network. Your out of network insurance may cover as much at 60-70% of the costs. Call your insurance company to find out what your out of network reimbursement is for physical therapy.
