Phase 2 Face Home Care, LLC works closely with family members, hospitals, physicians and other members of the medical team to develop a care plan that will best meet the needs of the patient.
You, your family and our staff will work together for a common goal while receiving face-to-face treatment throughout your phases of care. We understand that not one care plan fits all. Daily services can include anything from dieting, hygiene, cleaning, and supervision. We will take the time to get to know you because the needs of our patients and their families will always be our first consideration.
