Services
For over 31 years M & R Orthodontic Laboratory has built a reputation on consistency, quality, and reliability. M & R Lab offers a selection of over one hundred and fifty types of appliances, all fabricated from the highest quality materials. From the simple band and loop space maintainer to the most complicated functional appliances, we are able to fulfill the needs of your practice.
This warranty is VOID under the circumstances of damage due to patient abuse or distorted impressions. After the warranty has expired, all appliances are subject to repair or remake charges. Practices are responsible for accurate models, wax bites, and completed prescription forms. All appliance are guaranteed to fit the models sent in by the practice.
All payments are due by the 15th of the month in which you receive your invoice. Payments on invoice balance not received by the 20th of each month will receive a courtesy call as well as assessing a $5 late fee plus a monthly finance charge of 1.5% which will appear on the next invoice. If there is ever any issues with the invoicing, please call the office immediately at 985-871-9893.
Place RX & the models or impressions in appropriate packaging with our mailing label attached. If you are not using our prepaid U.S. Postal Service mailing labels, be sure to ship packages to the address below. Payment Policy: Invoices are mailed or emailed on the 1st business day of every month. Please see our full billing and payments policy under our "About us> Billing and Payments Policy" section of our website.
We know how important it is to have a set date to receive your appliance and we do our best to be timely. All fabrication times given are for amount of days IN LAB and exclude weekends and holidays. The day AFTER we receive the model or impressions with a completed prescription starts day 1.
Please be aware that we will call if we have questions or concerns on any cases and that until we have all questions or concerns resolved this does delay fabrication time resulting in longer completion dates.We ask that if you receive any notification from us regarding a case to please get back with us as quickly as possible in order for us to be timely with your case.
