Services
Recovering from a recent hospital stay following an injury or illness can come with many challenges. Often times, a family member, based on their experiences caring for loved ones at home, believe that certain changes require hospitalization. However, while true for the elderly living at home, one of the advantages of having your loved one in our care is our ability to respond and manage virtually any need they have - now and in the future.
I am so glad my father is a resident here. Words can't express how grateful I am to the entire staff for ensuring that he stays engaged, healthy and happy. Thanks for all that you do! Mom is so happy and thankful that we moved her here, and we're very thankful to the staff and the residents for being so understanding and helpful.
My name is Andre E., I reside at the Meadowview Rehabilitation and Nursing Center as a long term resident. I am writing on behalf of the new ownership of Premier Healthccare. I would like to commend their efforts in making Meadowview Rehabilitation and Nursing Center a better facility! They are changing lives giving the residents hope for a better life.
Whether as a result of a trauma, disease, stroke, surgery or chronic condition, when mobility, function or cognitive abilities are impaired, rehabilitation is often the pathway to recovery, renewed independence and quality of life. We have been serving the health care needs of seniors in our area for more than two decades.
Prior to a resident's discharge from our facility, our rehabilitative professionals make sure they're ready for the challenges involved with returning home or to an alternate level of care. In addition to outlining personalized exercise plans and guidelines for safely performing daily routines, our therapists will make sure each patient can manage stairs and other obstacles related to returning home.

I’m thankful for the CNA that was caring for my loved one but after this visit I plan to come more frequently very sad situation I’m very well informed about long-term care facilities and the care that’s should be provided , I’m familiar with calling the department of health and the ombudsman It’s better to do that then to leave reviews. I pray that this facility get it together for the sake of the residents.Families really have to be proactive and visit as much as possible or your family member will decline, I’m trying my best to take into consideration Covid but I’m a very disgusted

If you're thinking of sending anyone 2 Meadowview don't!! There were only a couple good AIDS and a couple good nurses all the rest don't care, you have to wait a long long time that you're call Bell answered. They don't listen to you when you tell them you're sick, they try to put you on a schedule for changing you! They berate you and scold you like you're a child. I was their only a week and I have been in the hospital over three weeks because of this place! Do not send anyone to this place.

Meadowview Nursing & Rehabilitation Center is a wonderful facility. The staff is very attentive and caring, from the front desk to the housekeepers, nursing and the Administrator. From start to finish they take care of their residents like they are their own family and make sure to keep their loved ones updated on all the care they are providing. Their social services director, Stephanie, is outstanding and takes such pride in all the residents she helps. Highly recommend this every evolving facility!

My grandfather has been a resident there for 8 years and my family and I are not pleased with the system, the staff, nor nurses. No one wants to work right. The nurses don’t take care of your loved ones as they should and whenever a resident needs assistants the nurses will take a long time to assist them. My father was sent there after he had Covid. Nameless/ faceless staff, who all appeared to have little regard for proper care. Once he arrived, we immediately knew this was not the place. He was there little over a week, and we spent the entire time working on getting him transferred.
