Families Together Adult Day Health Center is a basic medical day program where disabled adults and seniors can go during the day to have their activities of daily loving cared for, rehabilitate any physical and mental health issues, and take part in therapeutic activities. Families Together ADH is your home away from home.
Services
Seven years ago, Daniel Estevez, began working in the child and senior welfare field, supporting families, young people, and seniors. Daniel supported families and young people who were involved with the Department of Children & Families and supported seniors who were involved with different Senior Care agencies in Worcester county.
Our Adult Day Health Care is a medical model adult day care program that serves adults with special physical, mental and psychosocial needs. Members are encouraged by staff to increase their self-esteem, social interactions and independence. Members who participate in our Adult Day Health Care Program will be able to remain with their caregivers and strengthen ties to their community.
Daniel Estevez is the current owner of Families Together, LLC and co-owner of Synergy Wellness Center. Daniel's passion for caring and his vision for well-being are the fundamental factors that encourage him to provide the best services possible for each individual at Families Together and Synergy Wellness Center.
If you are 18 years of age or older and have a physical or mental condition which impacts your daily life, you may qualify to receive our adult day health care services. If you have MassHealth Stadard, Commonwealth Care Alliance, Fallon Navicare, or Senior Whole Health, your insurance may cover some or all of our program costs.
