Windsor Woods Rehab & Healthcare Center provides coordinated, comprehensive services to meet the care needs of the individual resident and patient. The interdisciplinary team members will collaborate with residents, patients, families, and staff to develop an individualized plan of care for each resident and patient.

We have developed successful clinical systems including specialty programs and best practice programs which enable our nursing teams to achieve enhanced resident satisfaction.Our Administrative staff, including our administrator, business office manager and admission/marketing staff work together to manage the day-to-day operations and business-related activities.
You can ask to see or get an electronic or paper copy of your medical record and other health information we have about you. Ask us how to do this. We will provide a copy or a summary of your health information, usually within 30 days of your request. We may charge a reasonable, cost-based fee. You can ask us to correct health information about you that you think is incorrect or incomplete.
List not all inclusive - inquire today for how we may provide services unique to your situation. The facility is staffed with licensed nurses 24 hours per day and a team of licensed Physical, Occupational, and Speech therapists.

The interdisciplinary team works with the attending physician to develop an aggressive rehabilitation program and ensure the complex medical needs of the patients are addressed and a comprehensive plan of care is developed to help these patients return to their normal level of functioning as quickly as possible.
Certified Nursing Assistants (CNA) are the heart of our general nursing and restorative care programs. They focus on providing care and comfort while striving to enhance the joy and dignity of life. Our restorative CNA assists residents in achieving their highest level of practical functioning, while our psycho-social CNA helps our activity director in presenting an array of social events and recreational activities.

I STAYED AT WINSOR WOODS
AND WAS A VERY VERY GREAT PLACE THEY TRULY
CARE ABOUT THE RESIDENTS AND HAD FANTASTIC STAFF. THE MANAGER DONNA WAS ALWAYS ABLE TOT HELP AND RESOLVE ANY ISSUES I HAD ALL THE NEGATIVE REVIEWS ARE NOT TRUE I STAYED 8 MONTHS AND ONLY HAVE POSITIVE THINGS TO SAY THANK YOU FOR MY STAY

I was a resident here. After a horrible car accident. I had several severe issues. I could not walk and I couldn't really do anything for myself. After 3 weeks in there care. I was transferring by myself. I was up and about. There staff is amazing by far. There nurses and CNA's went above and beyond for me. Expecally there nurse LIZ she honestly is the best I've ever felt with and truly cares for the world she does. I appreciate everything this facility had done for me.

This is an awesome skilled nursing facility! The staff truly care about their patients and provide great quality care! Administration team is very involved and supportive of the patients and families and will help address any questions or concerns that families have!

My uncle stayed here for rehab. Staff was super nice. Nurses was great. Super smart. Forever grateful for the team who helped my uncle and made him feel at home!

No! Just no! If you have a loved one and don't want to lose them, stay far away from this place!!! My father is dead due to their negligence. There is nothing anyone can do to bring him back, but here they stand! Stay away from here!!!

My grandmother was a resident for 6 years. They showed nothing but love and kindness. I have been to facility many times and never saw anything wrong. No bugs or dirt. Before covid the would take them on field trip to restaurants and play lots of games. Their covid numbers were really good in the beginning. They only got high now in the last few weeks due to lack of mask and vaccination being used in that area of Florida. Unfortunately the whole state is suffering.

My grandmother was a patient at this facility for two weeks and it breaks my heart the way she was treated. She was denied of phone privileges and the use of a bed pan. She was given pain meds without considering her weight and daily intake. When I went to visit her, she was so out of it and loopy from the meds. In addition, She sat in her own waste for hours before she received care. Five days went by before she was even offered to clean her dentures and a clean wash cloth. My personal experience was horrible as well. Nobody calls you back and when you do receive a call, you're provided with erroneous

Do NOT leave your loved ones here; specially if they have Alzheimer’s. My dad was moved here from the hospital for rehab with a pelvis fracture and in a matter of two weeks he fell three times because he was drugged ALL the time. His clothes were lost and they were new, his dentures were lost and he lost weight as if he was never fed. Instead of improving, he went backwards. When my mom decided to take him out of there he had clothes that were not his, filthy socks and his nails were grimy. They do not have enough personnel to give patients the treatment a human deserves. The hospital should

DO NOT put your loved one here. Money was stolen from my mother's purse while there in rehab for a few days. Director/Administrator (you know who you are) does NOT follow up with us about a "report" that they supposedly filed (we were never given copy of or report # as proof that one was filed). Director/Administrator refuses to talk to me and says she needs to talk directly to my mother but yet doesn't follow up with her. It's been about 2 weeks and we still have not heard a word about a status of when my mother will be getting her money back that was stolen. We decided it was necessary to file

This place is so gross we are work to get my Uncle out of here. To start we saw a big cockroach in his room. Therapy only happens about 30 minutes a day the rest of the time the residents are just sitting around in wheelchairs doing nothing. They over medicate the residents, they were giving my Uncle Tylenol 3 with codine and he wasn’t in pain. They just wanted him to sleep. They actually ground it up and put it in apple sauce. Do not put your loved ones here.
