Concepts of Health Care began in 1985 when the founder, a registered nurse, was working in the discharge planning department of a large local hospital, and discovered how many arranged home care plans failed once the patient left the hospital.
The reason for these failures occurred when many of the agencies failed to follow through with the promised home health aide services needed to provide the care for the patients, once the patients arrived home from their hospitalizations.Thus the beginning of Concepts of Health Care, Inc., an agency that follows through with its promises.

After my father's hip replacement, my mother was struggling to care for him at home. Concepts of Health Care provided my mother the help she needed to keep Dad home, where he dearly wanted to be while he recovered from surgery. The services truly enhanced both my mother's and my father's life. The caregivers and nurses were wonderful.
Whatever option suits your needs, a registered nurse will do an initial assessment of the client/home and an individualized care plan for the client is established. The assigned aide(s) will then follow the prepared care plan for all activities of daily living for the client.
This includes, but not limited to: assisting the client with personal hygiene and mobility, meal preparation, keeping the home environment clean and safe, doing laundry, providing companionship and supervision, accompanying the client to any appointments, and alerting the nursing supervisor immediately of any changes in the client.
