Santa Cruz Post Acute is a premier post-acute rehabilitation center located in the heart of Santa Cruz, California. We provide personalized rehabilitation and nursing services. Our healthcare professionals foster a caring and supportive environment, and we strive to help our patients meet their greatest potential to ensure a better quality of life.
State of the art therapy services with Renowned Physical, Occupational, and Speech Therapists. As well as providing skilled nursing services, our social services staff serve several functions at Santa Cruz Post Acute, but our primary role is that of the patient advocate. Social services is here to follow our residents from admission through discharge.
While we've tried to provide a comprehensive list of questions we're asked on a regular basis, there might be something we may have missed. The absolute worst. A family member was there for rehabilitation. What little was done accomplished nothing. As soon as the insurance said they weren't paying anymore, she was out the door. Wouldn't feed my dog the food they serve. I hope I never have to go there.

Being a patient here the nursing is great but the facility has a big problem with their pharmacy they are dealing with I have been here 10 days and the very first night I had to wait 12 hours for even an aspirin and now after 10 days they cannot get the medication delivered it has been over 48 hours since the doctor ordered it I wouldn't stay here if you are at all interested in keeping you health

I give SCPA very high marks. I am caring for two people over 92 and have seen many SNF sites. SCPA has been the most attentive with both bathroom needs and with providing companionship. Choosing a skilled nursing facility is always a difficult decision to be made at a stressful time. I'm so happy that our choice worked out so well. Thank you so much SCPA!

My mother died in this facility after a 5 month stay. After 3 weeks of "searching", the facility has not been able to locate any of her personal property, namely her glasses, watch, hearing aids and charger, bible and clothing. The rechargeable hearing aids were less than a year. I can only conclude that the hearing aids and charger were stolen since they cost $5,000 and the remainder of her personal belongings were thrown away. Heartbreaking in the light of only being able to see her through a window 3 times due to Covid.

I am writing a review for Michelle Bayuga. I am currently a short term resident. I've been here 4 years+. I have watched Michelle grow from a receptionist to a fabulous Social Services rep. She helps us with our appointments, rides and various needs. I'm heartbroken to find her leaving next week. She should have been promoted. I don't think they know that this diamond in the rough is blooming into a beautiful flawless diamond. She's going to school and will soon have her degree. I wish Rusty could see all the hard work behind the scenes. Many of us have gotten stuck with Hearts

I've partnered with Rusty for many years now. Working with close to 250 nurses every year, Rusty has been instrumental in adding professional insider view to many of the nurses. I've seen those who have had the opportunity to meet with him thrive tremendously. As for my nurses who choose to go and work with Rusty after school, I've received nothing but positive back from them. I recommended highly as a business partner

My 92-year old mom has been there for two weeks rehabbing from a fall. The staff has been wonderful. Caring, responsive, competent and hard-working, especially the folks in rehab who are very dedicated and professional. The facilities are simple, but clean and well lit. The food is ok, nothing special. Parking is definitely hard to find.

A relative has been health challenges. After treatment in the hospital she had been transferred from Dominican Hospital to Santa Cruz Post-Acute. Over the next nine weeks, family members and myself had made many visits.We have been so very impressed with the professionalism and coordination of care. The kind and caring people at Post-Acute helped our family deal with our emotions when our relative was so very ill. They were also were alert to changes in her condition and made decisions to send her back to the ER at various times. If they hadn't been so alert, she would no longer be with us

This place is deplorable! Dirty, old, disorganized, noisy, nursing staff lacks empathy and necessary training, etc. My family member was discharged with active C-Diff and a Stage 4 pressure wound. We were told that insurance would no longer cover "unnecessary treatment." Two minutes after discharge, I forgot to ask the Dr. (Dr. Potts) a question, and went back to ask him. His reply to me was, "He is discharged, and no longer my patient. You will have to get him to his primary care physician and ask them." ARE YOU KIDDING ME??? We were given NO anti-biotics for the infections

It is a horrible place. Dark and grim, smelly, old building filled with the psychiatric patients. When the evening sets, they start screaming, running and acting. Then, the place resembles Bedlam. It is not possible to sleep. The level of noise from the patients, the loud music they play day or night, room call bells and machines approaches the industrial level requiring ear plugs or muffs to survive. Toilets are routinely unavailable. The place has numerous good nurses, but they cannot change how it is run. The management is highly incompetent and cares only about extraction of money. The place

My mother has been in Santa Cruz Post Acute (formerly Kindred Care) for about a week now after a bad leg break. So far, I am impressed with the staff and facility. I had an ex GF and her mother who worked there before . Sadly, her mother became a patient and had cancer , so the outcome was not good . Having worked in the Fire Service, I have seen my share of facilities, both in Santa Cruz and over the hill. This is a far cry better than what I have seen before . Mom complains because "there is too much food and it is wasteful"...also, she doesn't like anything she doesn't cook

SC Post Acute (formerly Kindred) has the most caring and kind staff who give their best to take excellent care of their patients. The staff is cheerful and encouraging. The administration process was fairly easy and efficient. Their administrator, Rowena is kind, competent and simply the best!

What is needed though, is a renovation. The building is dated and needs updating with some thoughtful touches to make it feel more like home. Even new flooring, painting and furniture would be transformational. Since they already have great nurses and staff, the renovation would be the cherry on top
