Services
The transition to a nursing home can be a major change for both the resident and the family. Skilled nursing care at Hearts & Hands Post-Acute Care & Rehabilitation Center offers around-the-clock care for persons who require the services of professional nursing staff or are unable to live independently.
Our experienced rehabilitation team is committed to providing outstanding results and excellent quality of care at our Rehab center located in Santa Cruz. Residents are empowered through individualized programs to reach goals of increased independence and ability. Resident families are also encouraged to participate in therapy sessions, caregiver education, and training.
Hearts & Hands is proud to offer individualized outpatient therapy to our community. Each patient will be evaluated based upon his or her diagnosis, rehabilitation, and personal goals. Our staff members will strive to improve the quality of life for each patient who enters our doors through our unique land-based and aquatic therapies.
Long-term care is provided in different places by different caregivers, depending on a person's needs. Most long-term care is provided at home by unpaid family members and friends. It can also be given in a facility such as a nursing home or in the community, for example, in an adult day care center.
Hearts & Hands palliative care program treats residents suffering from serious and chronic illnesses. It is one of the best outpatient hospitals in Santa Cruz. Our care focuses on helping each resident gain the strength needed to carry on with daily life. We strive to facilitate relief from the symptoms and stress of the serious illnesses our residents suffer with.

Absolutely the worst place I've ever experienced this staff hardly speaks English and there is an average of four to five hours response to any request. I've waited for days from medicines I'm one of the healthiest people in here that's walking and speaking and just watching people in wheelchairs getting ignored I can't believe this place is even in business I fear for anyone who has a loved one here you need to be on top of what they're doing I've been giving

I very satisfied with the place is clean and the service, professional stuff for rehabilitation my brother is there he had a stroke, he couldn’t walk a move some parts of his body, now he is better in shape than before and walking , now they are working in his speech. The personal administration are very helpful in anything, careful in this time of COVID-19., highly recommended.

Our mother was recently admitted to your facility and the subsequent 3 weeks has been a stressful combination of her children dealing with her new situation & living conditions, while simultaneously needing to go through her 25 year, 2 bedroom apartment, saving, storing, and disposing of everything. Additional facility regulations (quarantine) prohibited us from decorating (much) her initial (temporary) room. These cascading whirlwinds together created an adverse effect on us all, and the emotional toll and feeling has been borderline overwhelming.

Thanks team Hearts n Hands for your great service. Your care and dedication towards patients is remarkable. Therapy goes above and beyond. I've consulted with a number of various physical therapy facilities, and this is by far the best. The nursing team were outstanding. The incharge nurses are all incredible. With every need, they was helpful and polite.

Very hostile work environment. Poor patient care. They always have zero Perry care items to properly clean the residence. None of the bed controls work so sometimes the patient is stuck in the same position. The administrator is a young guy who has no idea what he is doing. It seems like this place is all about the money. I would not send my worst enemy to be cared for here. This is coming from a previous employee. They always have registry in the building because no one wants to work there full-time.

It is a very accessible and very professional place. My aunt was there before she died, and it was very pleasant for her. They always kept me up to date on the problems my aunt had. The staff was always very friendly and helpful. I never had any problem with the service to my family member.
I recommend that you stop by to see the building and its great staff, in fact, I take a great memory of how important the reign was for my aunt's last days.

I can't thank Hearts and Hands enough for taking care of my friend's aunt so well. She was admitted here about a year ago and I have only seen them improve. They have the best staff one can ask for. Not only do they help my friends h her med administration but also grooming as well. The activities staff also paints her nails. Also like many other facilities the food that they provide is not at all bland. It is in fact very nutritious and tasty. They take special consideration into the kind of diet a person needs.
