This can lead to depression, continued cycles of crisis, and readmissions to costly rehabilitative programs.

Neuro Community Care LLC (NCC) has proudly served the needs of those living with brain injury and other neurological conditions since 2011. During this time, the highly trained NCC professionals have devoted themselves to providing individualized supports in a cost-effective and meaningful way. Neuro Community Care supports over 500 clients nationwide.
Below is a pre-contract application which will assist us in determining more about your agency and the potential to provide services for our clients. If you do not provide these trainings, are you willing to implement these trainings for your contracted staff with NCC? Has anyone affiliated with your agency ever been employed, contracted, sub-contracted or worked in a volunteer capacity with any of the following organizations?
I started Neuro Community Care (NCC) in 2011 to provide support for clients and families who often feel overwhelmed when dealing with the complicated issues associated with brain injury on their own. NCC recognizes the complex, lifelong needs of persons with brain injury and their families and the challenges they face.
Neuro Community Care serves clients nationwide who have a brain injury, spinal cord injury or other neurological conditions. Adherence to substance abuse policies - not actively using drugs/alcohol during service delivery. We accept new clients from direct inquiry and also by referral from physicians, attorneys, case managers, health care professionals, insurance representatives, family members of persons with brain injury, and other sources.
Neuro Community Care's traumatic brain injury (TBI) and PTSD case management services are designed to meet the unique needs of each client and support their progress toward real-life changes. Case Managers work collaboratively with clients, caregivers, and providers to develop goals, access services, and provide advocacy.
