Hillcrest Commons Nursing & Rehabilitation Center has been part of the Pittsfield community since 1995, providing short-term rehabilitation, long-term skilled nursing care, and specialized dementia care in Western Massachusetts. Situated on a hilltop in a residential Pittsfield neighborhood just five miles from the New York state border, Hillcrest Commons Nursing & Rehabilitation Center is easily accessible to major local highways and hospitals.

We understand that the need for post-acute care is often unplanned, and we offer admissions seven days a week for the smoothest possible transition between care settings. A personal care assessment determines the appropriate level of care and establishes preliminary goals. Hillcrest Commons Nursing & Rehabilitation Center offers a range of care options to suit your needs.
Our on-site rehabilitation team includes physical, occupational, and speech therapists, and specially trained therapy assistants, all dedicated to helping you regain strength, mobility, and independence, and achieve the best possible outcome. Our experienced team of rehab professionals is here seven days a week, as ordered by your physician.
We start planning for discharge upon admission, creating personalized treatment plans that implement physician's orders to meet personal goals. Our 24-hour skilled nursing teams provide the care and attention needed to maximize recovery, and a family meeting is held within 72 hours of admission to review expectations and address any concerns.

This place needs to be shut down ASAP. I'm currently involved in abuse case regarding my mother. Where her life was threatened by a LPN. She's witness out right assault and they told her she Hallucinated it. It was investigating over night,no report filed. My Mother reported the Threat and it was never reported to the State. If your loved one is there. Get them out now. This place is extremely dangerous and the State is now involved.

I was a resident at Hillcrest Commons Rehabilitation Unit and was surprised how nice and accommodating it was. The Physical Therapy Dept was amazing, the rooms were clean, the staff was very attentive and the food ok. The management & Guest Relations Coordinator was very accommodating.
I would recommend Hillcrest Commons.

I rated this place an hour ago and after reading all of the negative feedback I chose to rate it the lowest possible rating. If as much time and effort was put into making this the best place possible for residents and staff ( upper mgt. noninclusive, as they are the main problem here ) this will be a great place to spend one's final years. I noticed that to the only pictures of this place, all two of them are outside the facility. One is a picture of the sign out front and the other is a picture of Valentine Rd. (nice blacktop HA!) and that's it. I challenge them to take pictures of the patient

This nursing home is the worst. They are always under staffed, there fore patients aren’t getting good care at all. The employees they hire seem unmotivated and under trained, that is very unsafe for our loved one to have to put up with. They treat there residents like they are in jail. It’s disgusting and I just want everyone to know to not put your loved one here.

Coming to Hillcrest had ;mixed feelings for me. the first was oh no whats happening. then after the first few days I found that the staff, from aides to nursing were here to help me to get back to a point where I could go back home with the ability to take better care of myself. the environment was clean and spacious. the food was so much better than I had . the care was much more conditioned for each individual than the hospital. I am grateful to the staff and all who worked with me to make my stay comfortable. I highly recommend Hillcrest commons to others who need a place to recover from multiple

My daughter had been there and she had never gotten treated so badly anywhere. Couldn't get meds when needed, never got what she ordered for meals. She has a feeding tube and didn't get fed the right amount. Too many things to list. If you have a choice where to go pick somewhere else. Maybe they were over worked I don't know, but you treat patients with respect.

If I could rate this place negative stars I would. This place is hell. Hell for the residents and hell for the people visiting their loved ones. My great grandmother whom I've been extremely close to my whole life was on her deathbed. We had to ASK a nurse to check on her. We had to ASK the nursing assistants to change her. My grandmother died that night. They didn't waste any time throwing away and giving away her belongings. Awful facility.

My husband was there for 6 weeks. Nursing care was good, CNA staff were nonexistant except for necessary things, food was AWFUL,
200 dollars of his clothing was lost and no reimbursement was forthcoming. PT and OT staff were very good thank goodness and helped him to get out of there more quickly.
He had been there before in 2013 and had recieved excellent care for the 10 months he was there. What happened???
