We understand that the need for post-acute care is often unplanned, and we offer admissions seven days a week for the smoothest possible transition between care settings. A personal care assessment determines the appropriate level of care and establishes preliminary goals.
Our dedicated core team includes nurses and nursing assistants, full time nurse practitioner, rehabilitation therapists, activities staff, a dietitian, consulting physicians, and our Medical Director.These specialists, along with our entire staff, work together to provide a place that feels like home.

Our on-site rehabilitation team includes physical, occupational, and speech therapists, and specially trained therapy assistants, all dedicated to helping you regain strength, mobility, and independence, and achieve the best possible outcome. Our experienced team of rehab professionals is here seven days a week, as ordered by your physician.
We start planning for discharge upon admission, creating personalized treatment plans that implement physician's orders to meet personal goals. Our 24-hour skilled nursing teams provide the care and attention needed to maximize recovery, and a family meeting is held within 72 hours of admission to review expectations and address any concerns.
At Mt. Greylock, we focus on the individual, rather than the illness. Our residents and patients receive personalized around-the-clock nursing care and supervision as needed, including assistance with activities of daily living, such as dressing, bathing, eating, and getting around. Orthopedic rehabilitation for joint replacement, knee replacement, hip replacement, fractures, and degenerative joint diseases.

My mom has been living at Mt. Greylock Extended care for over 2 years. She initially was there for Rehab after a hospitalization , and did very well making a lot of progress, but it was just not safe for her to be living at home with me any longer. She needed more care than I could provide for her. She was transitioned to long term care which was, looking back, harder for me to accept than it was for her. She seemed right at home from the start.
I visit my mom often and always find her to be comfortable and well cared for. The nursing staff , activity department staff and rehab keep me updated

My wife was in last year and the care was excellant on every level from the Entery door to the Maintenance staff , Nursing , CRA 'S , PT , OT , SPEACH , kitchen , entertainment , laundry etc. My mother has been there since the middle of last year and it remains the same level of care . It is NOT your old nursing home by any streach ! Brovo to such a wonderful ,careing staff !

I just want to let people know how blessed we were to have connected with Mt. Greylock. Our family went through a very difficult time.My father in law is disabled and needed to be admitted for long term care as his condition became too difficult to handle at home. At the same time my mother in law was very sick and was sent to Mt Greylock for rehab after surgery and remained there for 3 months. The staff of Mt. Greylock ALWAYS went above and beyond to care for my in- laws with the best available nursing, PT, OT, etc! It was a very stressful time for us trying to manage health their concerns and
