We understand the strong environment of care that you've created for your residents and aim to match that same approach through our visiting primary care physicians, physician assistants, and nurse practioners. We meet with your residents in their apartments on a regular basis providing the preventative care needed to reduce the need for emergency room visits or hospitalizations.
Your residents deserve the most seamless coordination of care possible. This is why our primary care team has invested the time necessary to build strong relationships throughout the Twin Cities community to facilitate streamlined care for your residents. Our goal is to become your resident's trusted source for many of their care needs.
We understand that your resident's care concerns are not limited to a 9-5 window, so we don't limit your access to our team either. Your residents or staff members who reach out to us will have the ability to speak with a live member of our team outside of normal business hours. Additionally, we provide a detailed emergency contact plan for every patient so if an emergency occurs, you know what next steps to take.

I have worked with TCP for almost 9years. I am a nursing director at a very large assisted living and memory care. I can say that my residents have never had better care! Steven Atkinson and Anna Dudzik have been the most attentive practitioners I have ever worked with! I would trust them with my own family members. Truly a care driven company!

I am a nurse at North Ridge asst living and TCP is the main provider that come here to see residents. All of the providers that come here are very nice and knowledgeable. I always have positive interactions with Gina S. Erika G. and of course my favorite is Emma D. Always a quick response if there are any issues or concerns. Today I had a super positive interaction over the phone with Bryce, who helped me get back into the portal. He laughed and didn't make me feel stupid, I am very computer/technology challenged. I can not say enough great things about this company and their staff.

No communication whatsoever! Portal does not work. New patient registration does not work on website. There is no way to communicate what is happening.....UPDATE: They have reached out to me. They are working on updating their portal to communicate but in the meantime they did give me an email which is good and have been on top of this and things are being solved. So instead of one star I will give them a 4 star
