Heart & Soul LLC provides Congregate Waiver Services for individuals who have been diagnosis with an intellectual disability. Services are offered in both residential and community based environments with training and assistance provided in areas of personal care, socialization, communication, community integration, behavior management, health, safety, domestic skills, money management and other daily living skills.
Each individual shall receive a provision of supports based on the assessment and development of their Individualized Support Plan. Our Staff consist of QMRP, QMHP, & LPN'S and together we have over 20 years of experience. We offer a team approach in working with residents with dual diagnosis.

Our goal is to deliver high-quality, integrated care to our individuals, as is reflected in our vision - Keeping for People a Quality of Living. Making a Difference. The Program Administrator or Program Administrator Designee of Heart & Soul shall ensure its quality of services. The Program Administrator or Administrator Designee is responsible for the day-to-day direction of Heart & Soul's Medicaid-funded programs.
In this program, individuals will learn to budget and use their money effectively and efficiently. During this session, individuals/residents learn what medication(s) has been prescribed, its treatment purpose and its side effects. Medication is administered by a registered medication aid as prescribed by physician.
We provide education, training and community support programs that contribute to individuals becoming valued and productive citizens of the community. We provide services that promote individual empowerment, self-esteem, career development and personal growth. We work to decrease the high rate of penal recidivism by providing on-going positive and structured community services and supports.
Heart & Soul ALF offers a safe, wholesome and homelike environment for ambulatory residents. We are staffed with qualified professionals, who are CPR/First Aide certified. Staff is on duty 24hrs a day, 7 days a week. There is a Licensed Practical Nurse (LPN) on duty four (4) days a week in case of minor non-life threatening emergencies.
Heart & Soul LLC "COMMUNITY EXCURSIONS" is a Psychosocial Rehabilitation Program committed to providing innovative services that allow each individual to participate in various meaningful activities and accomplish independent living skills in various community settings. To provide supports that help individuals feel and become empowered to make choices that promote self-direction, self-esteem, and personal accomplishment.
