A leader in clinical eye care, Dr. Carlyle Haywood, OD offers comprehensive eye exams for adults and children, pediatric eye care, exams for eye emergencies, and so much more. We provide treatment for a range of conditions, including glaucoma, diabetic retinopathy, dry eye, cataracts, and macular degeneration.
Answer all questions as fully as possible and submit your completed form at least the day before your office appointment with us. This will save YOU valuable waiting time. Don't forget to bring ALL medical/vision insurance cards and your Driver's License or Picture ID. If you don't provide us with the necessary insurance information then you will have to pay out-of-pocket for the services we render to you.

Could not ask for a better eye dr. for my family! He has seen my 13 yr old for the past 2 yrs and my 11 yr old. They take the time to listen, to learn each of them. I have been to 2 different eye drs for my children and they got frustrated with them, they didn't care about there feelings or nervousness, was NOT ok with me going back with them😳!(and before you ask, was it an eye pediatrician? One was and one was kid and adult)

I definitely recommend this practice. I’ve been going since I was younger, even my mom went when she was a little girl! Their customer service is amazing & I’m always greeted when I walk inside the door. They also have a nice range of selection of glasses for those who has Medicaid! I 100% recommend anybody to go! I do drive almost an hour to this office, so trust me it’s great practice with great people that cares! See you soon in 2 years to get my eyes checked again lol.

Nose pads on my glasses came off so I called Dr. Haywood's office to ask if they could repair them. Not only did they have me come in as a walk-in right away, but they fixed them on the spot! I was not a patient of theirs but the next time I need new glasses they're my first choice! Highly recommend!

Dr Haywood office is the best doctor I have ever been to. I've been to several in the past but when I went to see Dr Haywood it was the most relaxing atmosphere that you could experience. from walking in the front door to seeing the doctors, picking out my glasses. I wish I had been going there for my whole adult Life but I will continue in the future to see Dr Haywood and his staff as they take care of their patients Thank you Dr Haywood and all who work in your office.
