Crestmont North Nursing Facility is committed to providing premiere healthcare service in Lakewood. Whether you need long term care, short term care, skilled nursing, or outpatient therapy, we strive to have our residents get well, be well, and stay well. Joseph Coury founded Crestmont North in 1986 and is currently owned and operated by his son, Elias.

And is committed to keeping the family tradition, of providing patient-focused care, alive and well. To provide an experience to all our resident/clients which values their choice, safety, dignity and quality of care.
Services
Crestmont North Healthcare focuses on long-term care but also provides short-term, respite and specialized care as needed. As a 24-hour skilled nursing care facility, we offer individualized plans of care for all residents and begin to plan for discharge as soon as short-term residents arrive. Recovery from significant surgery, injury or illness may create the need for rehabilitation.
At Crestmont North our residents have a choice. Residents who have a private physician can keep their current doctor who in turn works with Crestmont North's medical director and nurse practitioner to create a personalized care plan. Residents can also choose among our attending physicians or medical director to follow you and your care needs.
Crestmont North's expert staff provides the tools necessary to recover quickly. Our nursing and medical professionals provide supportive care and comprehensive services so residents can live independently and return to the home they love as quickly as possible. For families taking care of their loved ones at home, it can become challenging, overwhelming, and stressful.
Encompassing the entire continuum of care, Crestmont North Healthcare Center provides long-term care to individuals requiring assistance with daily activities such as bathing, feeding, dressing and administration of medicine. Crestmont North specializes in the transition from a home or hospital to a nursing facility.
Crestmont North's specialized rehabilitation services are delivered to individuals with short or long-term illness or injuries who are unable to return to daily activities without the continuing support of nursing, medicine or therapy. We believe in helping individuals achieve the highest level of independence possible, and our physical, occupational, and speech/language therapy programs are provided with this goal in mind.

My grandma has been here for a while now. I’ve always appreciated the friendly staff, they’re always willing to go the extra mile.
The one thing I would like to note that separated them from others when looking originally was it is very clear, they go above and beyond when it comes to cleaning. My grandma actually jokes that she thinks they do more cleaning than any other activity there.
Overall awesome experience highly recommend

This place is horrible, lost my moms POA paperwork, Louise especially was like ohhh her poa was never uploaded to the computer and its nowhere to be found, just like my moms medicaid paperwork. The nurse had to go to the back and get it off her chart. This nursing home needs to be investigated and closed down period.

I originally was at Crestmont North for another reason. I was treated fairly and with kindness. Crestmont was well maintained and was taken care of properly.Today all the place is improving an they taking care or it a little bit better than when I first came here in August 2018. However it still needs more updates and improvements to this place. For example the carpeting needs either to be cleaned with rug cleaner that someone uses it to wash with a carpet cleaner or have the carpet replaced and taken care of, for example if someone spills juice of any type they should clean it up right away before

The people that work here are not skilled or educated. The food they feed the patients is inedible. They are rough with the elderly
The place is dirty and run down. It is really crazy how unbelievably lazy the staff is. DO NOT PUT A LOVED ONE HERE. THEY WILL NOT BE WELL CARED FOR.

It is incomprehensible how unhelpful the staff is to work with. From the social worker to the nurse to the receptionist, they are all rude. You receive a different story each time you speak to someone and nothing gets accomplished. They are not knowledgeable of how the system works. Extremely unprofessional and by far the worst nursing home I have dealt with in my 20 year career.

I visit Paula Dupree on December 16,2015 for more than 3 hours and in the evening @3:57pm and her nursing assistant never came to ask if she needed anything or go to the bathroom. I took her, her peri area was so strong of non cleaningness. After I washed her, I looked for a clean Depend and found a soiled in trash bag in bottom drawer of her dresser. Her son Donald Dupree, Jr. is aware. I am a cousin to both of them. This care is highly unacceptable.
