Members of our interdisciplinary team of trained medical professionals are carefully selected for their ability to work with others to provide compassionate care. Working together, the team reviews each individual's treatment history and goals to create a customized care plan. Each resident's short- or long-term care is customized for their individual needs and preferences, and delivered around-the-clock by trained nursing professionals.

Therapy services are available to all of our qualifying patients 7 days a week in our spacious therapy gym by a team of dedicated therapists.
Renew Dialysis provides staff assisted home hemodialysis treatments inside Southern Hills Skilled Nursing & Rehab Center. Using the most advanced NxStage equipment, the treatments are tailored to help patients get the most out of their time at Southern Hills.

DONT SEND PEOPLE YOU LOVE HERE!!! The aids as well as ministration have no respect and do not care about the well-being of your loved ones. Well visiting relative has seen multiple negative behaviors from staff. Complete facility is very inconsistent.

Grandma was left to sit in same position to the point of infected bed sore. Did not remove or change catheter resulted in uti. Not a good situation for her as the bed sore required surgery, infection to the BONE!!!! Gram shared she enjoyed bantering with staff and they were friendly. That may be so, but she was neglected none the less. Not right!

This place is a JOKE. My aunt got placed her briefly. My father and I showed up to visit to only find out it’s by appointment. No problem. Even though the lady was very rude in letting us know. And I had asked for a number and her response was “maybe if they get to it tomorrow”. So we had zero way of communicating with our loved one. I had called the next day for the same woman to have yet another attitude on the phone with me. She had told me that my aunt got her number yesterday. As if I was supposed to know. She gave me the WRONG NUMBER. She has such an attitude she couldn’t even listen

I give Southern Hills 4 ⭐️. When I needed rehabilitation because of a accident that left me with one arm & both legs broken they came to the hospital to talk to me. When I got there the first thing I noticed was how clean the whole facility is. During my 4 month stay I was treated with respect and kindness. The Rehab therapy dept. staff are nice and relentless. The facility is like a tight knit family. I took away a star because of call lights not being answered in a timely fashion.

In all of my years in the nursing field, I've never in my career have ever experienced any facility as subpar as this one! Petty employee's and administrators speak to staff so disrespectfully! There is no problem solving just new rules everyday to benefit the people who truly do nothing and literally watch you brake your back working! From being micro managed for non sense via cameras, to having no staff or the supplies you need to even do your job was just HORRIBLE! I'm always positive...but this place drained my spirit! I feel so sorry for the residents who have to live there!

I transferred my mom to Southern from another nursing home. Although she was there under hospice for only 4 months I want to thank The Director of Nursing, Janet and Administrator, Diane for sitting at my mom’s bedside during her last hours until hospice and my family could arrive. You both showed so much love and compassion to my mom and our family. Thank you and your entire staff.

Terrible experience. Lack of medical care caused my mom to end up back in the hospital numerous times. Did not tend to her needs when she had an eye infection and uti, so both conditions worsened even after literally begging nurses and administrators to act upon her needs. She became severely dehydrated and hospitalized because of the neglect. The fact that she has a kidney transplant made it worse because now her kidney has acute damage to it due to the dehydration. Moved her out of the facility and would never recommend. They need to staff more nurses that can actually give the residents the

I understand that the administration has completely changed. I certainly hope so. My mother was in the long term care for 7 months and that was 7 months too long. The place was filthy. I had to clean fecal matter off her bathroom floor and toilet more than once. The food was mostly sandwiches, hotdogs, and soup. Mom's call button was on the floor behind her chair constantly. And when these issues were brought to the attention of the administration, they were always busy, or would "take care of it". The final straw was when I found my mother sitting in diarrhea and the aide said she was going to

My children's father has been in this nursing home for 3 years and in that time I have never once encountered an unkind unprofessional or less than helpful person. The facility is very clean the cafeteria dining room is awesome almost like a restaurant in fact. I was there recently for lunch and they served lunch in five courses feeding the residents that needed help. It is an awesome place. I would highly recommend it. I am a retired paralegal and I've seen and heard a lot regarding nursing homes and this one is a 10, bar none. 😀

I was told when I interviewed that all employees are happy there, and they have little turnover. From the first day I started working, all I heard was how unhappy the staff was, there was almost no positive feedback from employees. Staff was being called off because of the number of residents, with NO consideration for acuity, which was frustrating. 1 Nurse would care for an entire unit which could be as many as 20-30 patients, with no consideration for acuity. Orientation consisted of 3 12 hour shifts and then on your own. I was scheduled every other Saturday and Sunday, but they didn't tell me
