Countryside Health Care of Milford is committed to providing health care services that meet the needs of the community. The Skilled Nursing level of care as defined by the Center for Medicare and Medicaid Services is for patients/residents that have had an acute episode that requires medical management from nursing and/or rehabilitation services.

These individuals may have the potential of returning to the community or to a more independent living environment. The Long Term Care level of care as defined by the Center for Medicare and Medicaid Services, Division of Health Care Quality, is for residents who cannot safely return home and are dependent upon others for care and quality of life.
Services
The Rehabilitation Unit is directed by Dr. Alan Glaser and is staffed by a team of caring professionals made up of Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, Social Services, Dietary, and Activities. The team reviews each plan of care weekly, in cooperation with the patient and family.
The Occupational Therapist will work with the resident to improve his/her independence and safety in performing all of the Activities of Daily Living (or ADL's). This includes the most effective, safe, and energy-efficient way for bathing, dressing, grooming, feeding oneself, and performing simple homemaking tasks such as food preparation, if needed, for each individual.
The Speech-Language Pathologist, available on a consultant basis, provides therapy to those patients with speech impairments and post-stroke difficulties, assesses memory loss, teaches coping strategies, and sets up alternative communication systems if needed.

The speech pathologist also addresses swallowing difficulties, identifies problems, recommends required tests, follows-up with physicians, initiates therapies, and provides patient, family, and staff education.The speech pathologist participates in discharge planning and makes referrals to appropriate agencies and therapists in the community.
The role of the Social Worker is to provide emotional support and counseling to residents and their families throughout their stay at the facility. Assisting residents and families to cope with the limitations an illness can place on one's lifestyles is central to the social workers' role. This is done through counseling and education.
The Activity Department provides an ongoing program to meet individual needs and interests with opportunities for social, emotional, spiritual, therapeutic and recreational growth during short-term and long-term stays at the facility. The words "thank you" are not enough to say how much we appreciate what the staff has done for my husband Benny and me.

We come for visiting but doesn't permission to visitors right now.I've been there couple times before covid 19 comes....Is very well clean...nice people work there.This place is beautiful and have a nice paisage outside.

Went there for rehab from Milford Hospital. I’ve never had to be in a rehab, so of course I didn’t know what to expect. First thing I noticed was how clean it was. Maybe not a priority for some but a big one for me. I was in there for a total of 12 days. The care I received from day one was amazing. CNAs Nurses, physical Therapist, housekeeping , every one, every shift, wonderful! My only complaint was the food, it was awful.. the good news? My family and friends made sure I ate! My mother was here for 4 weeks. This was her second time using a rehab facility. The first rehab was AWFUL!! She was afraid to go in again. This facility is top notch from cleanliness to staffing. There is plenty of help, very professional, friendly and caring. My mother actually enjoyed her stay and my family and I appreciate the care she received.

Absolutely disgusted with administration! Family member has been there few times for rehab- wanted him to return after recent hospital stay and was told by administration that the antibiotics he is on are to expensive and they would not make enough profit on him so they could not accept him! WHAT?!?! No beds a available at any local facilities so we are now trying to find somewhere to place him
To deny someone the care they need because you won't make enough money is absurd!

We placed our dying mother at Countryside after she and we endured 5 days at a hospice house in Worcester. My mother received more compassionate care at Countryside in the one day she was there, than the entire 5 days at the hospice. The nursing and administrative staff at Countryside could not have been nicer, more professional and more accommodating to us, especially as it was literally a "last minute" request to find a room for my mom. Countryside was clean and bright and there was plenty of hustle and bustle by the nursing staff so that we never felt alone or neglected. If you

The best care I have seen in my lifetime and was in the medical field for over 20. Years. If you have a loved one please don’t hesitate to put them in the hands of this place. Will give you so peeve of mind. Right now I am 76 years young. So should I ever need this kind of care I feel so confident to have my family help me to reside in this place to give my the best life of the time I will have left. My mom stayed here for a couple weeks when she fell and broke her pelvis last year. They treated her very well. This place is very busy, they always have something for the patients to do. Sometimes it could seem to be a little hectic. The Nurses and Aides are very nice and very helpful. They even changed my moms room when she said it was to noisy in that room.
