Services
The rehab team utilizes specialized equipment to assist each resident in maximizing their functional gains during the rehab process, often decreasing the length of stay. Our 24-hour care staff is dedicated to meeting the needs of each patient and resident, providing excellence in an array of services.
The Pathway to Home Program is designed to assist patients with a safe transition back to the community as soon as possible. Upon admission, a member of our Interdisciplinary team will schedule a Partners in Care meeting with the patient or their family; we will discuss the care, goals and projected stay for the resident.

My mother, Judith Reed, was at this facility and had to be taken to Citrus Memorial per her Cardiologist because of water build up in her body. This water retention was because she was not given her water pills the whole time she was there. She was transferred there from Moffitt to get her leg strength back after being in Moffitt for 2 weeks. She was supposed to be on a vegetarian diet, but they always gave her meat. The first day of therapy the therapists came to take her to the gym. They ask why she wasn't up and if the CNAs had been in yet. When advised they hadn't, they went to tell them to

When I got there I could stand and take a few steps. They had to take me out in an ambulance, to the hospital, unable to move my legs. They were ordered to give me a water pill. I was there 13 days before they gave me one. By then my lungs and stomach were filled with air, which affected my heart. Stay away from there.

Fantastic facility. They go way above and beyond residents and family's needs. The BEST in the area. Wonderful and caring and very professional staff, nursing, and Doctors. Spacious rooms, facility, and grounds. Daily activities to keep residents entertained, educated, busy. Dietary an A+++. Food and nutrition remarkable. Highly recommend for short term rehabilitation and LTC.

I recently had to find placement for my mother after she fell and broke her ankle. I live out of state and was very nervous because of the recent increase in COVID cases. The admission team was very helpful in answering all of my questions and when she admitted the Nurse on her unit called to go over everything and update me. My mother had a great experience and was able to rehab and travel to come and live with me. I would definitely recommend Diamond Ridge Health & Rehab.
