I am a clinical psychologist and a Fellow in the Academy of Cognitive Therapy, an experienced therapist with twenty years in the field, and I specialize in treating anxiety problems at SohoCognitive, my downtown NYC psychotherapy practice. Sophisticated CBT is my experienced, practical approach to doing therapy, and it's an outgrowth of my unique background in the field.
People usually call me seeking help with anxiety that is getting in the way of their functioning their best at home or at work. For example, people call for help with really specific anxieties like panic attacks; fears of flying, riding the subways or elevators; fears about speaking in meetings; as well as more general stress and anxieties like being overwhelmed at work, having insomnia, chronic worry, "a nervous stomach, " or extreme shyness.
I originally trained in a specialty known as "psychodynamic" psychotherapy, which is the open-ended, exploratory talk therapy historically practiced in New York City. While this approach does provide support and can help patients gain insight into the origins of their problems, as a therapist, I grew increasingly impatient with its glacial pace and limited impact on my clients' day-to-day lives.
The basic premise of cognitive therapy (also called CBT, or cognitive-behavioral therapy) is that one's interpretation of a situation, rather than the situation itself, determines the feelings and behaviors that follow. These interpretations take the form of super-fast, knee-jerk fleeting thoughts, called "automatic thoughts."
I work with clients on a wide range of issues, from relationship problems to work stress, from family issues to career goals, from life transitions to making big decisions. When you've got significant stressors going on, sometimes it's hard to function or feel your best. Insights about a problem are hugely important in therapy, but with my Sophisticated CBT approach, my aim is to put the insights to work.

Dr Melamed, to put it mildly, changed my life. I had never really heard of behavioral/cognitive therapy. I'm used to a good old psychotherapist. You sit there and rant about your mother and they nod sagely and give you drugs. I had an awesome psychotherapist growing up, but after a string of terrible shrinks in adulthood, I just gave up on therapy altogether.
But the truth is, I'm kind of neurotic and high-strung and anxious (hawt, I know). And after a series of itty bitty breakdowns culminated in a kind of major breakdown, I decided that I had to do something before I broke in two
