Our commitment to excellence earned a 2016 Silver - Achievement in Quality National Quality Award from the American Health Care Association/National Center for Assisted Living (AHCA/NCAL).

We are honored to be one of just 3 centers in Massachusetts and only 71 nationally to earn this recognition in 2016. Our spacious, secure dementia care unit, Longmeadow House, provides specialized care for those living with memory impairment such as Alzheimer's disease. We understand that the need for post-acute care is often unplanned, and we offer admissions seven days a week for the smoothest possible transition between care settings. A personal care assessment determines the appropriate level of care and establishes preliminary goals.

Our dedicated core team includes nurses and nursing assistants, full time nurse practitioner, rehabilitation therapists, activities staff, a dietitian, consulting physicians, and our Medical Director.These specialists, along with our entire staff, work together to provide a place that feels like home.
Our on-site rehabilitation team includes physical, occupational, and speech therapists, and specially trained therapy assistants, all dedicated to helping you regain strength, mobility, and independence, and achieve the best possible outcome. Our experienced team of rehab professionals is here seven days a week, as ordered by your physician.
Our Steps to Strength short-term rehabilitation wing provides a comfortable, dedicated setting for recovering and regaining strength following surgery or hospital stay. Our brand-new rehab wing features all private rooms with showers, specialized medical capabilities for complex clinical treatments, and our Steps to Strength Short-Term Rehab Gym and Therapy Room.
At East Longmeadow Skilled Nursing Center, we focus on the individual, rather than the illness. Our residents and patients receive personalized around-the-clock nursing care and supervision as needed, including assistance with activities of daily living, such as dressing, bathing, eating, and getting around.

My Mom went here for rehab after a hospital stay for pneumonia. Staff were very attentive, helpful and professional. They always came when my mother pressed the call button. And answered all my questions. Phone calls to my Mom went through seamlessly. I saw strong Covid 19 protocols in place. Just wished they dressed my Mom every day. I supplied plenty sets of her clothes, but had to specially request that she be changed out of the johnny. I guess a minor issue. She received good treatment there.

New facility, used old beds. Self pay for respite care. Deplorable. Food terrible, small portions, warm at best. Infested with ants on first floor. Love walking through a cloud of smoke from staff smoking in front of main entrance

I am happy with most of the card my mom gets at the facility. However, I have the hardest time talking to my mom on the 4 th floor. Which I totally blame on 2 nurses who are rude, unprofessional and liars. All the other nurses and sides are great! The 2 nurses are Susan and Israel. I have put formal complaints against them. They constantly try to prevent me from speaking to my 99 year old mother who has Dementia. Susan even tried to disguise her voice and tell me she was the Security Guard. Israel, will flatly tell me she doesn’t have time to talk to and doesn’t want to. Obviously, they should

I am a great and loving cna I worked there a couple of years ago I had problems with staff I love the job not the ppl the nurses want to be friends with the cna's so they turn a blind eye to neglect of residents and bad and nasty treatment of other cna's this is a very clicky place to work yes there are some great ppl here most are smile in your face and play perfetional but they are wicked ppl and I think bad behavior is rewarded GOD BLESS

***PLEASE DO NOT SEND YOUR LOVED ONE HERE** Staff very neglectful of patients, always on personal cell phones, patients basic needs (bathing, brushing teeth, etc...) not met. Staff very rude and confrontational when ask questions or asked for help.

Staff very very rude. Alarms always going off in multiple rooms & staff ignores to socialize and use cell phones. Seen patients fall several times in attempt to get up on own for restroom when nurses ignore them for long periods of time. Food is awful including bologna sandwich for dinner. Nurses & aides argue with patients & family and they are very mean & rude, hard to find someone in this place that cares. My family member didn't get bathed for almost 2 weeks. Facility smells like urine. Please save yourself stress & heartache and pick a different facility for your family member.

One of the worst facilities ever. My Mom stayed there and the nurses were so cruel, rude and down right mean. She couldn’t go to the bathroom on her own, she had to buzz for the nurse they would never come. She would have to go in her bed. She couldn’t even take a shower because their showers were not working, no water. The worst smell in the bathrooms, always backed up because the nurses would flush the under garment diapers in the toilet. When the nurses did come they would yell at her. My Mother shared the room with another patient and the poor woman would be screaming at night in pain calling

I was a resident at this facility for a little over two weeks at the end of September. My room was on their new rehab unit and I could not have been happier with the accommodations. The nursing staff was very attentive and the aides were always available when I needed them. My room was always clean and housekeeping would come to check in multiple times a day. Their rehab department is there seven days a week so I was able to receive rehab daily which helped get me home sooner than expected. I would highly recommend this facility.

My mother is a resident at the nursing home (for over 4 years now). They do a fantastic job taking care of the residents and make them feel like "Family" instead of just doing a job with warm body.

Please let me preface this review by stating that in order to rate this facility, you must highlight an above star. There should be NO stars. In fact, maybe stars to the NEGATIVE.
This nursing home, pathetically also stating it is a rehabilitation center, is the epitome of a dirty, disgusting institution, slightly reminiscent of Belchertown Institution, which was shut down years ago. This quasi nursing facility is everything but that. Having been there quite often, there are rarely any nurses on the grounds. In fact, the woman handing out meds one night, was from an 'agency'.
