The care plan and clinical pathway for residents participating in the CHF and Pulmonary programs is followed closely by the Care Transitions nursing and respiratory team.This group of clinicians works with the interdisciplinary team, including hospital partners and consulting pulmonologists on a daily basis to ensure high quality clinical outcomes, strong customer service, decreased readmission rates and shorter lengths of stay.

Through Touchpoints Rehab's comprehensive heart failure rehabilitation program, patients receive personalized care from a multidisciplinary team, with attention paid to maximizing the effectiveness of medication therapy, enhancing knowledge of chronic disease and increasing exercise tolerance. Care is holistic, personalized and specialized to the unique needs of each of our patients as they move through the continuum of care.
Pre-booking your post-surgery rehab will ensure you will receive the services of your choice. Planning your post surgery care ahead of time will reduce your stress and let you focus on recovery. You can pre-book for every level of care. Transition from hospital to post-acute inpatient rehabilitation; directly from hospital to home care; or directly to Outpatient Rehab.
Touchpoints at Bloomfield offers a wide range of sub-acute skilled nursing specialty programs designed to get residents back to health, back on their feet and back home where they want to be. Touchpoints at Bloomfield nursing staff is experienced in caring for complex medical needs beyond heart failure and pulmonary specialties.
At Touchpoints at Bloomfield our Social Services department is staffed with concerned, invested and dedicated professionals. This meeting and much of the team's work is geared towards a safe and successful discharge home or to the next site of care. Social Service team members work with residents on a one-to-one basis and serve as a liaison between staff and families.
Through its managed care centers, iCare Health Network (IHN) offers a range of services including medical, short-term rehabilitation, long-term care, behavioral health, substance use management, and memory care & support services.

This would help to increase patient satisfaction and decrease patients from feeling frustrated and diffuse irate behavior.

Cindy was excellent and helped keep our loved one there in touch with family during Covid. Through FaceTime, phone calls, and visits, she was always wonderful and accommodating!

I been a patient here for over a year the Administrator Heather is always smiling and asking how is are day going. The recreation staff lead by Cindy and her fabulous staff making it enjoyable to be here. The nursing staff along with the cna and pac are top notch.the cleaning crew is always cleaning to make the rooms nice.this is a really enjoyable place so if you are looking for a place for your love ones I recommend here.along with physical therapy headed by Ann and her staff are great the work you hard to get you better.

AWFUL PLACE! If you care about your loved one do not leave them here. The place wreaks of feces as they do not care for the patients I was a patient here for a year and was mistreated terribly. Avoid at all costs....notice that all the reviews are from insiders and staff that give it 5 stars...dont be fooled. This is NOT the case.

I have been a resident here at Touchpoints at Bloomfield for the past 9 months. I was in pretty bad shape. I wouldn’t give the staff the time, but they showed me they would not give up on me. I have been treated with much respect. No place is perfect. When I have a question the staff helps me. Dwayne in recreation has been a big help. He has giving me so much support. Rehab has been working so hard in helping me recover. The nursing staff continues to help me with anything I need. I’m thankful and look forward to returning home.

I've been working as a volunteer at Touchpoints in Bloomfield for 8 years. I help lead a ecumenical worship service at the facility and visit residents. I have always found the staff here professional and caring. There is a very active recreational program for the residents, so that they stay involved with one another and are motivated to be part of the community, not just stay in their room. I can't comment on the medical care since that's not my specialty. But there is high socialization of the residents that helps with their well-being and relationships.

My name is Cindy Austin and I am the evangelism director at The Church of Jesus Knows Best, Inc., in Hartford (700 Blue Hills Avenue). Our ministry has been affiliated with what is now Touchpoints of Bloomfield, for more than 20 years. I became familiar with the facility because my father was a former resident before his passing and I appreciated the special care they gave him during his extended illness. It was then that my family and I decided to volunteer our services in a meaningful way. This awesome relationship has continued for close to 30 years!

When I first was hired, I was working as a receptionist. After 3 months, I had the opportunity to work in the Recreation Department. It has been a pleasure to work with such a creative department. In the past, I have worked at other facilities and the programs that are provided at TAB don't come into comparison. It's a rewarding experience to provided activities for our residents.

I have been an employee at TAB for the past 30 years. The reason why I have been here for so long is Icare is a good company to work for. They have good benefits and treat the staff well. I enjoy working with the residents and keeping them happy. I have a great relationships with the management team. We are a TEAM at TAB.
