This facility needs to be reprimanded by the state immediately. I came to visit a family member during lunch time and to my horror, I walked into an unsupervised dining room with residents sitting with cold soup in front of them. Moments later an employee who later identified themselves as an activity aide, walked into the kitchen serving the main course with no gloves or hair net being used. Certified kitchen staff should be hygienically serving my loved one's food! Furthermore, there was only one nurse working two of the three units within the facility. She was frantically going between units
