You may either fax your completed patient form to us at 281-392-1125 or bring it with you to your appointment. In addition, please remember to bring your insurance card and photo ID with you to your appointment. We accept many of the major medical insurance plans, including Aetna, Blue Cross Blue Shield, Cigna, Community Health Choice, Humana, Medicare, Memorial Hermann, PHCS, and United Healthcare.

Made multiple visits for chronic eczema. long waits to get back to the exam room, extremely quick visits, not much interaction, seems he already has mind made up what he will suggest/prescribe before he comes in. the last visit however really upsets me. he finally tried something other than creams, gave me a cortisone injection. i paid my co payment and went on my way. couple of weeks later i get a bill stating i owe more money. called insurance company, they said the shot he gave me was coded as surgery!!! they gave me the correct billing code, i called the office and gave it to them. they said
