Woodhaven Center of Care Assisted Living Facility is the first Medicaid funded, New York State Licensed Assisted Living Program in Suffolk County. Woodhaven Center of Care is a 143-bed licensed Skilled Nursing Facility, as well as a 181-bed Medicaid funded, NY State Licensed Assisted Living facility.
Both are located on the same campus in Port Jefferson Station. We have been providing loving, dignified care to our residents for more than 40 years, with Long-Term Care, Sub-Acute Care and Short-Term Rehabilitation while being certified for both Medicaid and Medicare. Our diverse facility includes such programs as Rehabilitation Services, Social Work/Discharge Planning, Nutritional Services, Nursing Services and Therapeutic Recreation.

Woodhaven Center of Care Assisted Living Facility is the first Medicaid funded, New York State Licensed Assisted Living Program in Suffolk County. For over three decades, we have been serving our residents and caring for their needs to help them transition into their new phase of life with ease and comfort.
Woodhaven Center for Care Assisted Living Facility provides a long list of amenities to make our residents comfortable. Residents who are unable to perform daily tasks such as cleaning and cooking are accommodated by our staff and are able to spend time engaging in social activities with other residents.
Respite Care is care provided as temporary replacement of a live-in caregiver. Our Respite Care program can give you peace of mind to travel, while we care for your family member. The Suffolk County Office of the Aging has a program that provides payment for short-term respite at Woodhaven Adult Home.
Cancer is a chronic disease with intermittent acute episodes resulting in the need for care in various settings, including rehabilitation and long-term care. The multidisciplinary team, familiar with the patient's disease, treatment, and prognosis will integrate all aspects of care and will assist the patient through all phases of an exacerbation of disease or chronic disease.
Our nursing staff has the knowledge and compassion to serve the elderly, the disabled, and those recuperating from illness or injury in a resident-centered care environment. Nursing care is provided through an interdisciplinary approach to maximize the highest quality of care received by our residents.

After suffering My fiance was released after 3 months . No home care set up , I was misledcas to his abilities, hevfell twice within 24 hours when I called I was told that I knew what I was getting into. I trusted them to give me the tools necessary to take care of him. They were calling big talk that they would be there for us it was called a lie

Carol from admissions made my mother in law's transmission into the facility so smooth. I actually was able to sleep at night knowing that she would be cared for. The entire staff was extremely pleasant and attentive. It's not an easy decision to put your loved ones in a nursing home, but at least I know that she had the care she needed.

My Dad has received rehab services at Woodhaven Nursing Home on a few different occasions, the staff has always been 5 star quality! A few months ago my dad returned to Woodhaven he immediately called me to let me know that they had renovated the entire building and it felt like he was in a 5 star hotel.

The staff at Woodhaven Nursing Home made my mother’s stay there for several months, especially during these difficult times really wonderful.....The Care, compassion, kindness and dedication to their residents and patients is abundant,.....they always made her feel cared for and that she really matters.....showed her love and respect.... They made it so much better for her..... They kept her safe, involved and medically cared for so well.... the food and recreation departments are great also.......They all uphold their patients dignities and respect each individual.....The staff is always available

My experience with Woodhaven,
I love my Nana to the moon and back. I am an extremely thorough individual who puts hours into research before executing. I would not put the health and care of my Nana in the hands of anybody I wouldn’t trust to take care of me personally.
The professionalism displayed at Woodhaven was simply unmatched (in my opinion being compared to other nursing/care facilities). I was still skeptical at first but after asking existing patients and their families I decided that Woodhaven was the best for my Nanas happiness and health. She was actually willing to accept help

Woodhaven nursing home is one of a kind! the admission process went smoothly with Courtney who was able to answer any of my questions. I was grateful for how attentive and compassionate all the nursing staff was and the therapy team is so determined. They encourage you to get to a normal baseline and try to make it fun. I highly recommend this place!.

So thankful for this facility! The staff at Woodhaven have made me feel very reassured about my grandmother being there by treating her in a warm and supportive way. It's clean and bright and I highly recommend.

The Best Nursing Home/Rehab!! I’ve had the best experience with the OT AND PT staff, they helped me come a long way and they are extremely gentile and understanding and they helped me walk again and get me stronger so I can be who I was before and the recreation staff is awesome, they are always there to help you have fun, the nurses are wonderful, and so are the nurses aides. I would recommend this facility to a family or a friend in need! Maintenance is always on spot and so is Housekeeping. The nutritionist is very understanding about your dietary needs. I will definitely miss this place when

My grandmother has been living her for a couple of years now and I want to stress the word "living". They take wonderful care of my grandmother and they treat her with love, kindness, and compassion. The staff is attentive and goes above and beyond to make sure everyone is involved and doing well. They have recently renovated the building and it looks magnificent!!!
