Outreach Eye Care
Since 1978, Outreach Eye Care has provided exceptional on-site vision care to residents in health care facilities. We believe all people should have the best view of their world - regardless of any physical, memory, or behavioral challenge. Many elderly residents who live in nursing homes have unmet eye care needs.

Undetected eye diseases such as glaucoma, cataract, diabetic retinopathy, and macular degeneration are chronic and common. Our doctors are well versed in the surveyor guidance contained within the Critical Element Pathways, specifically the Communication-Sensory and Activities of Daily Living CEPs.
To assure an accurate and timely 'paper trail' regarding your residents' vision care services is available to you, Outreach Eye Care will:. Provide a copy of all scheduled services one week prior to visit to assure resident availability. Review with a member of your facility staff all provided vision exams, exam outcomes, and appointments that required rescheduling (due to resident's unavailability).
