The facility has been in business for thirty years, and we are undergoing some very exciting changes, and renovations.
We are a ninety bed facility, with both semi-private and private rooms. We offer long and short term stays, respite, hospice, and palliative care, skilled nursing and rehabilitation heartfelt care. The facility has a beautifully renovated dining and recreation room, attached to a sun room, which is every resident's favorite spot, to enjoy visiting with friends and family, or to just take some quiet time.

At Colonial Health and Rehab, we have created an environment which combines and celebrates these memorable aspects of life and creates new experiences for you.
Colonial Health & Rehab provides a full spectrum of long and short term nursing, rehabilitation, dietary, and social services as well as therapeutic recreation programs. Colonial Health & Rehab provides twenty-four (24) hour around the clock nursing care. Our team of professionals works with your physician to maintain the residents' highest practical level of independence.
Take I-395 to Exit 88 (Sterling/Oneco). Exit the Interstate and turn onto Route 14A (Academy Hill Rd). Coming north you will turn left, coming south you will turn right. Follow Route 14A, turning left at the first traffic light that you come to. Route 14A merges with Route 12 (Norwich Rd) here. Turn left at the next traffic light, continuing on Route 14A (Railroad Ave).

All of the good ratings for this place are from employees who are bribed to write good reviews in hopes of winning a gift card offered by management in return for a good review. This place is a sinking ship and won’t be able to operate like this much longer. Everyday every department works short and are expected to do things not in their job description due to the fact that this facility can not hold any employees. People are mandated every single day for 16 hour shifts where they will work short. The residents do not deserve this.

Residents are treated as family members. Multiple activities including games, religious services and entertainment are provided on a daily basis. Staff are rewarded periodically throughout the year with gifts of appreciation from the owners and administration staff such as pizza parties, ice cream socials, and activities during nursing home week. Administrative staff are helpful and can be used a resources. Residents, family and staff are encouraged to bring forth any issues or concerns so it can be addressed in a timely manner.
