At 4C, our fellowship-trained and board-certified Specialist physicians work closely with your primary care physician to help manage your care.
At 4C, we believe in being able to provide continuity of care when patients are admitted to Skilled Nursing and Acute Rehabilitation facilities after a hospitalization. Our Post Acute Care physicians provide care to patients at over 40 hospitals and facilities in the Phoenix metro area and collaborate actively with care management teams to improve care outcomes and patient experience.
