Our nursing program is designed to provide outstanding patient outcomes. Our skilled nursing team is trained to manage medically complex patient care and provide critical support for optimal rehabilitation. Each of our residents comes to us with very different and specific rehab needs. Our entire rehabilitation team is committed to creating individualized therapy programs that directly answer those specific needs.
Sub acute care is defined as comprehensive inpatient tracheostomy and/or ventilator care designed for someone who has an acute illness, injury or exacerbation of a disease process. It is goal-oriented treatment given immediately after, or instead of, acute hospitalization to treat complex medical conditions or to administer technically complex treatments.
Our experienced rehabilitation team is committed to providing outstanding results and excellent quality of care. Residents are empowered through individualized programs to reach goals of increased independence and ability. Resident families are also encouraged to participate in therapy sessions, caregiver education, and training.

My mom calls this place hell. She tells everyone she knows not to go there. She was there February 2021. She couldn’t sleep because it so noisy. Wake you up at 11pm to weigh you. Wake you up in the middle of the night to take blood. The meat they serve was a mystery. The vegetables and deserts were ok. The nurses were nice. There was no watchable programs on tv. It’s a rehab facility and all she got was 15 minutes a day. She was expected to do things herself.

This place is a complete joke! The staff is very rude and condescending what’s scary is the supervisor is just as rude. My Father was there for 6wks to rehab his back from surgery he said they would wake him up in the middle of the night poking him and moving him around while he was in pain. Rude staff horrible service and will lie to you in your face with a smile NEVER AGAIN!

So I call the facility to make a appt. see my mom ,they connected me to Activities dept. Told the young lady can I come friday she said yes,I said you need my name no we don't just come I said ok,drove 2 hours got there unable to see my mom,said only 3 visits a week ,well then why didn't you tell me that before telling me ok come at 10am,I didn't know other people came that week to visit her,but the facility did....so unProfessional people working there with no knowledge of # visits

Gladstone has been a wonderful place for my Mom and they have made me feel more secure having such a caring staff. The activities girls decorate for all of the holidays and have fun games for the residents. During Covid, they have been taking precautions to keep everyone safe and although it is challenging to visit, they have been very accommodating in letting me visit my Mom every week when it was ok to do so.

In addition ,I wanted to add that Carmen from the activities Department has been exceptional! She is so kind and compassionate toward my mom. I know my mom is in excellent hands when Carmen is assigned to her care.
Also Mitra and John are both Super Nurses! They have both been so professional and has provided the utmost healthcare to my mom! I can rest well at night knowing when they are on duty my mom is is perfect hands!

My grandmother who is 100+ receives excellent long-term care from the staff at Gladstone Care & Rehabilitation Center. I am so grateful they kept her safe during covid! Currently, visitors need to make appointments to see loved ones and this helps ensure that covid safety protocols are followed. The staff go out of their way to give my grandmother lots of attention, and our family is so thankful for all they do!

My Dad was transferred there from the hospital, even tho it will be hard to have him home. I don't trust this place, you have to make an appointment to visit at least 24 hours in advance and I understand they want to be safe but they have more precautions than the hospital and you can only visit 30 minutes and only 1 visit a day. Also you can't really get information regarding your loved one as well. Just everything is very suspicious . Do not recommend

I was just here for a week and a half but you can feel the positive vibes and friendliness of the staff especially Cassandra who is welcoming and cordial at the onset. The Physical Therapy staff here are very friendly, courteous, considerate and attentive to your needs. I would highly recommend GLADSTONE Care & Rehabilitation Center to anybody who has physical rehabilitation needs.

As a friend and family member of a resident staying at Gladstone Care and Rehab, I would like to write a positive review. On various occasions, Ryan Bautista, the Administrator, and other staff members, have gone above and beyond to make sure my loved one and other residents have what they need and get the care that they have come there for. I love their activities director, Irma, who always has something new for them to keep them active and occupied! This type of care is very much appreciated and puts my mind at ease.

As a family member looking for a skilled nursing facility for a loved one, we are looking for quality over quantity when it comes to finding the right facility. We were looking for a place that are compassionate and well equipped to handle different levels of care. We were so please at how clean and nice all of the staff were. This place is awful. I've been give the wrong dosage of my medication. 5 times to much. The showers are cold and dirty. The food is awful. When your diabetic. They'll give you a fruit plate. Go figure. If you go out to a doctor or dialysis. If they forget to pick you up and u call the facility sometimes they dont answer the phone. Which is a common problem here. Call goes unanswered as well. The staff lies to you. Unless you speak up people go unshowered. People are not walked or taken outside. Mattresses are dirty, ripped and worn. Laundry gets lost. No follow up on patients doctors

First off, this place is horrible. They are so short staffed here. It’s impossible to get a nurse to help my Mom. The only good thing about this place is the therapists that come to do physical and occupational therapy. The doctors come once a week to check on my mom. The nurses are so short staffed, my moms diaper gets checked twice a day. It’s so sad. I would recommend going to a different rehab center. They don’t have 24/7 care, and they only have one CNA at night and 3 LVNs taking care of 20+ patients.... and this place is so expensive too!! 5k per month for assisted living and 14k for

My father in law fell and broke his hip. This center is very attentive. The staff was nice, professional and accommodating. The PT staff helped him get much stronger. He was regularly showered, shaved and changed clothes. He loves the food at this place!!! He fell again and we are at a new facility and it's terrible. We are currently trying to get him back in here. Only down side to this place is not enough beds! Sometimes you don't realize how good you have it until
