This is determined by whether you have insurance and the type of plan you have. Because plans vary so much, you should call the number on the back of your insurance card to find out about copays, coinsurance, deductibles, etc. All payments are due on the day of service. D., is a licensed psychologist at Lake Normal Psychological services and has over 25 years of clinical experience. Dr. Sadoff received her B.A. degree from the University of North Carolina at Chapel Hill in 1990 and her doctorate in clinical psychology from the University of Tennessee, Knoxville in 2000.
In order to prepare for our first session, it is important that new clients download and complete the following paperwork. Please review the following documents: Services Agreement, Notice of Privacy Practices, and Consumer Rights (Medicaid only). Print and complete the Client Data Sheet, sign the Signature Page and Authorization Form (if applicable), and bring them to the appointment with you.
