Board-certified physicians on staff at Northwestern Memorial or Advocate Illinois Masonic. Our staff member will request your driver's license or other government issued i.d., and your insurance card.

Michigan Avenue Primary Care is in-network with most major insurance companies and accepts the following:. Additionally, we will accept any other PPO plan not on the above list. However, if we are not in-network with the PPO (not one of the companies listed above), the patient may be responsible for a larger portion than if they see a physician that is in their network.

Never got past the waiting area so I can’t judge the quality of the doctors, but I had a terrible experience with the front desk staff.
First, I was booked with a male physician even though I requested a female. I called back and rescheduled, but they didn’t cancel the first appointment, and I had to argue to get out of the $100 no show fee for their mistake.
I tried calling because I was running about 5 mins late, and I wanted to see if they had any parking tips, but no one answered. I made it into the office (where 2 people were kicked back, not answering phones) 7 minutes after my 11:30

I've been seeing Dr. Mathew for the last 6 years and I always trust and respect her and the staff there. They've given me excellent medical advice over the years, and everyone there takes into account the whole person (lifestyle, diet, stress, etc.) not just your symptoms. I also love that I can get an annual physical AND my annual women's exam there at the same time. Great doctor's office all around, and I highly recommend!

It feels strange to rank physicians online, but my experience at MAPC has always been absolutely outstanding. I'm treated with respect. The physicians are excellent, and when my primary doc is out, the alternate physicians or nurse practitoners who help me out are always very helpful. They put me at ease, explain procedures, and etc. Highest rating!

They were so great! I missed my train and arrived 15
minutes late and figured I'd probably have to
reschedule, but I was lucky and Dr. Salvia had time
to fit me in that day and she was FANTASTIC! She
really cared and listened to my concerns and

I am a new patient at Michigan Avenue Primary Care. I was very happy with the practice as a whole. Zach was my medical assistant and he was very nice. I was evaluated by Lester a PA in the clinic and he was very professional and thorough. I had X-Rays done while at the office and the tech was very nice too. I am not from Chicago and recently moved into the downtown area. I am glad I found a practice that is so close! Good visit and glad that I picked Michigan Avenue Primary Care.

Long process to find a new doctor that started with health insurance + Zocdoc. Decided to go w/ Dr. Mathew after positive public feedback. Scheduled annual screening.
Everything went smoothly. Appointment started on time, 5 min for vitals taken by a nurse. Then 5 more minutes for the doctor to review vitals and get to work. Dr. Mathew addressed the questions I had, and provided lab orders to take to an in-network lab. Overall good experience.
Pricing is publicly unavailable for services, if you have insurance provide that otherwise you will be billed at the time of registration.
