You will receive a thorough evaluation to determine the cause(s) of your pain. We will review your medical records, obtain a complete history and physical and then design a treatment plan for your specific needs. Interventional Spine and Pain continues to provide the most advanced diagnostic techniques and treatments of back pain through comprehensive physical examination along with spinal pain mapping procedures.
Proper Co-pay amount (this will be a specialist visit, so please consult your insurance card for the correct specialist co-pay). NOTE: Interventional Spine and Pain does not provide disability evaluation or approval.

Dr. Zeballos, Dr. LeDoux, Dr. Walters and Dr. Michels will provide a complete evaluation, diagnosis, treatment plan, and if needed interventional procedure with follow up. Our team of physicians will be there from the beginning to the end. Their approach to pain management is a team process, and the patient has to be proactive along with the treatment plan.

Great service from the doctor. He was very knowledgeable and we reviewed my MRIs together which was helpful. Unfortunately, I had the same billing experience as another reviewer. I received an out of network bell 2 years late...with no hopes of a discussion with my insurance provider at this point. They do tell you that it is an out of network provider, but the billing office delay seems somewhat strategic. They treated me several time over a course of a year, but I wasn't made aware of the charges until June 2021. The services occurred between 2019 and 2020.
