For your convenience and to save time when you arrive for your visit, we have created these forms for our patients to be able to download prior to their appointment. You can type directly into the document (you must save the PDF first) OR you can print out each form and fill them out with a pen. Don't forget to bring them with you.
If you have a preference, please let us know. If you are comfortable seeing more than one of our providers, please check as many boxes as you like. While electronic communication is a good fit for some questions and answers, we might find it best to discuss your particular situation in our office. We will let you know and discuss your options with you.
Please note that you must provide the patient's account number in order for the payment to be applied to the correct patient's account. If the cardholder's name and billing information are different than the patient's information, please provide information for the CARDHOLDER so that we may properly authorize the payment.

A board certified dermatologist since 1998, Dr. Paige B. Camp completed an internship in pediatrics and a residency in dermatology at Medical College of Georgia.

Both are specially trained in the treatment of skin cancer and reconstructive surgery to repair the wound after the cancer has been removed.

The staff, both medical and administrative, are always attentive, friendly and helpful. My appointment went quickly, with almost no wait time between checking in and being called back. My concerns were checked and addressed efficiently; I can’t imagine going anywhere else for my dermatological needs.

I went to Marietta dermatology for a full body scan and was nervous putting it off as I hadn’t had one in years. I was a sun worshipper in my younger days!
Everyone from check in, the nurse who brought me into the room, the amazing and gentle doctor to checking out-they could not have been friendlier or more accommodating -they all went the extra mile!

I have used Marietta Dermatology for years. The doctors are great & the front desk employees are efficient & friendly. The reason I did not put 4 stars is that my appt was at 9:30. I arrived at 9:20. I was not seen until after 10:00. Maybe a staff shortage due to the Holidays, Dec. 28th, so I am not mad, just a little miffed.

In mid October, I tried to get an urgent appointment with the dermatologist I’ve been seeing for over 30 years & was told that they couldn’t get me in until December. When I told them it was urgent, they offered me no alternative, such as to be put in a cancellation list or anything else. When I said” I guess I’ll have to find another service, I was told, “Fine, go ahead!” So I called Marietta Dermatology who is only one block away from my previous center. They scheduled me quite quickly. I arrived at the office at 1:40 for a 2:15 appointment, knowing I would have paperwork to complete
