Country Villa Assisted Living was designed by long-time Freedom resident, Chad Reader, and his associate, Mark Hartzheim, based on a need for a local, rural and community-centered approach to assisted living.

Many of our residents were either born, raised, or worked on Farms in Freedom and the surrounding areas.

Services
Our Pulaski team at Country Villa Assisted Living has a broader approach to assisted living. Our Pulaski location was founded in 2016. Visit us today for a FREE tour! My nursing career started in 2000. I started during high school as a CNA in a local long term care facility. I later went onto school for nursing.
Country Villa Assisted Living amenities include private apartments with a private bathroom and individual heating and cooling controls. We encourage you to make your room as comfortable and home like as you want. Our support staff takes care of cleaning, laundry and other housekeeping chores that grow more difficult with age.
We also offer respite care if a loved one needs 24/7 supervision while a caregiver is out of town. We will temporarily admit them for 1-3 weeks for a flat daily rate. The rate includes all amenities listed above, along with any personal care services and medication management (based on availability).

My dad resided at Country Villa for 4 years until he passed away. His level of care was beyond excellent. Very caring staff. The buildings are bright, well decorated and very clean. Lots of fun activities and of course dad always looked forward to Happy hour. I couldn’t recommend a better residence.

The Country Villa has a very warm family feeling. Everyone is given the opportunity to participate in the activities each day which provides the residents with many opportunities to develop their cognitive, social, emotional and physical growth. (as my Dad would say we all have a lot to learn and it should not stop until the day we come to meet… our maker in heaven.) Country Villa is a safe and nurturing environment, and provides a positive and productive relationship with the families. My mom loves the care she has been getting and the staff is friendly and professional.
