Our original Appleton location opened its doors with open hearts in 2005 followed by our newest facility in 2008. Our Appleton memory care facilities have 38 apartments. Each resident has their own private room with a half bath. In each of the two wings, you will see the circular motion of our rooms, enabling staff to meet the needs of each resident.
If you are searching for a quaint, cozy place to call home, our Menasha location is a perfect fit. Our Menasha homes feature a layout of two side by side ranch style houses that provide quality care to only 7-8 people per residence. Our Menasha homes are intimate, peaceful, and comforting. Neither of our locations are intimidating because we do not believe in large facilities.

Heartwood Home’s intimate, secure setting has given our family complete peace of mind. The owners and staff are truly caring people, providing for their resident's physical, social and cognitive needs. The family-like atmosphere has helped our Dad feel comfortable. We went from worry to relief when he moved to Heartwood. They not only care for their residents, but families too, making sure we are kept up to date with health care changes and making helpful suggestions when needed.

A little over two years ago at the end of December, my parents moved into Heartwood Senior Living. I had spent two weeks looking at many facilities in the Appleton area. Both of my parents were at different stages of dementia, my dad at the beginning stage and mom more advanced. Mom was 92 and Dad was 93. They were inseparable after well over 60 years of marriage. Jason, the owner, went out of his way to make sure they were able to stay together. They were given two rooms next to each other so a door could be put in allowing one room to be a bedroom.

Heartwood Homes took care of my brother for 6 years. Prior to that, he was in a facility in Illinois that although caring, had so many residents that there was little personal caring.
Jason and staff always had my brother’s best interests in mind. They made him feel comfortable and actually happy while he was with them. Jason was a great source for information and guidance in managing my brother’s care for 7 years, through hospice, and ultimately his passing.
In his final days before passing, his cognitive abilities were greatly diminished but he actually said that he knew he was well cared

For the past three years, my father Chester, was a resident of Heartwood Homes. The decision to place my dad was difficult but we soon realized that our choice of Heartwood Homes was the very best. For three years the staff loved and cared for my dad and in doing so became part of our family. I want to thank and praise Heartwood Homes and the caregivers for making my dads transition/death a time of love, respect, kindness, dignity and understanding. A year after our mother was diagnosed with Alzheimer’s, my brothers and I realized she was no longer safe living independently. Heartwood Homes Senior Living is an exceptional assisted living facility in Appleton, WI. Heartwood Homes gives excellent loving care to all residents. Jason Schmitz, owner of Heartwood Homes, is himself a Registered Nurse and therefore, knows the care that is necessary and the concerns of the residents. The facility is always maintained in a clean and orderly manner. The staff is excellent, always available to care for the residents and provides much comfort in dealing with difficult situations and still maintaining a positive attitude in dealing with the various lifestyles of the residents.

An amazing memory care community for seniors! My mother in law was in Heartwood Homes until she passed away in November 2019 and we are so thankful for the caring staff and wonderful support that she (and our whole family) received from Heartwood Homes. In addition to the staff, the home is just beautifully decorated and so comfortable; we would visit for hours at a time and the staff was just wonderful to us all and made us feel so welcome. Before my mother moved to Heartwood Homes my family and I were providing the care for her that she needed. It was very important to us that we moved our mother into an assisted living truly specializing in memory care that made her feel like she never left home, and that’s what we found at Heartwood Homes.
