Legacy Heights Nursing and Rehabilitation Center is a modern facility taken under new ownership and renovated in 2011 to meet the growing and changing healthcare needs of the area. We provide professional, long-term care and rehabilitative services.

The main objective of Legacy Heights is to provide nursing and restorative care in a warm home-like environment that will encourage and enable each resident to achieve the maximum level of activity and self-determination.Residents of the Legacy Heights community can select either private or semi-private accommodations.
We provide Long Term Care services including both private and semi-private rooms and specialty accommodations for the bariatric community. Transportation is provided to and from medical appointments made by the facility. We make sure our residents are kept busy with a wide variety of social activities and other options available for them to enjoy.
The Activity Directors at Legacy Heights assess resident needs and develop individualized activity plans. Holiday celebrations, birthday parties, board games, Bingo, exercise, arts and crafts, religious services, story hour, and daily newsletter and television viewing are all part of the diverse activity program at Legacy Heights.
Each resident is different and requires different levels of care. It is up to you and your loved one to decide when the time is right to transition into a nursing facility. 2. What personal items can be brought to the nursing home? The rooms at Legacy Heights are uniquely designed to provide an atmosphere reminiscent of home.

This is a good place for families to live their last days out. Going to visit with them in the outdoor area is nice when weather permits. The outdoor area cant be beat.

They treated my mother with the utmost respect and concern. I would've liked to have been kept better informed, but I always knew they were taking the best care possible of her. When it was time for her to pass, all her favorites came in together, gathered around us and bid her a fond farewell. The staff is wonderfully personable, with few exceptions.

I couldn't have picked a better care facility for my after surgery rehab. The entire staff are amazing and the wound care specialist is by far the best there is. The compassion for the residents is outstanding. I would definitely recommend Legacy Heights and should I ever have a need for this type of care again I will definitely come back here

I faced racial discrimination and wrongful termination at this facility. You would think a facilty who has paid two fines within the last two years and not to mention stopped receiving Medicare/Medicade in 2014 wouldn't dare to even discriminate but they did because in their eyes I was just a "black girl" who couldn't afford a lawyer but they were wrong. Imagining coming to work at 2:53pm thinking your going to station 3 to see your residents that you've grown to love and adore but your suddenly for the first time on station 1 where your not familiar with the residents or with how things work on
