We anticipate the continued need for this unique and convenient way to provide quality care and look forward to connecting with our patients and community in person and virtually to accommodate therapy needs and preferences.Provide our expertise in injury evaluation to find out the cause of your pain or injury.
Services
We at Performance Physical Therapy are now able to offer Telehealth Physical Therapy services to our patients. This involves one-on-one video conferencing with our physical therapists from the comfort of your home. With some exceptions, our therapists are able to offer most of the benefits of a regular physical therapy visit while you do not have to actually come in to the clinic at all.
If it is your FIRST appointment, please arrive 10 minutes early to fill up the necessary paperwork, etc. It will expedite the process and will give you the most value for your visit with your therapist. Your first visit will be focused on the evaluation of your issue as well as some form of treatment related to your condition.
Please take your time to complete the new patient packet and review our Privacy Practices and Financial Policy prior to your initial appointment. We still would like you to arrive 10 minutes early to your scheduled appointment time. Feel free to call us at (360) 714-0870 with any questions or concerns.
Thank you for choosing Performance Physical Therapy for your musculoskeletal and orthopedic care. We accept most medical insurances. If you don't see your insurance listed give us a call to see if we are contracted with that insurance company. It is always important to check with your insurance company to verify your physical therapy benefits and find out if you will have a copay and/or need authorization prior to your PT visit.
Direct access allows the patient to be evaluated and treated by a Physical Therapist without being referred by a doctor or other healthcare practitioner for musculoskeletal issues. Sometimes direct access may be referred to as self-referral. YES! Physical therapists are trained to recognize "red flags" that may signal the need for more invasive medical intervention.

I started seeing Mark just before vacation that was going to entail a long flight. I had been suffering with sciatica and I was worried it was going to be a miserable flight. Literally after one treatment, my symptoms subsided so much so that it didn't bother me the entire vacation. What a relief that was! As I am an active person, I continue to go for treatments to keep things at bay. I have significantly less issues between appointments and progress every visit. I would not be feeling this way if it weren't for Mark and his excellent care! Thank you!!

Couldn't ask for a more professional and friendly team. They not only guided my rehabilitation, but they educated me and helped me build a daily/weekly routine to maintain my new (correct!) lifting mechanics.

After years of dealing with adductor/hamstring and groin issues, I was recommended Mark Jamantoc by a friend. Mark is a solid practitioner and after approximately 6 sessions I feel the best that I've felt in years restructuring old scar tissue. I maintain a very active lifestyle and know my body well and it was great working with someone who could listen and work with me in stride. Over the last year I have occasionally had to stop my chosen activity (running, lifting, biking etc.) due to pain and dysfunction, but over the last 2 months I've seen great progress with no issues. It is obvious that

I’ve went to Performance Physical therapy for close to a year to rehab my torn ACL. Katrina and Brian were so helpful! They’re positive energy and well planned schedules for me made rehab a better experience. Not only did they help with my strength but as well as my confidence in my knee! I now play college football and wouldn’t have been able to do it without their help!

Thank you so much, Mark J, PT, for you meticulous work with our son's tibial fracture. He is a passionate golf and basketball athlete and was able to return to his activities due to your diligent work with him. At six months post procedure, he competed in the state golf championship round, at 1 year post procedure, he is the leading scorer of his high school basketball team. He came out of his injury grateful for his health and thrilled he is able to compete again. We are so grateful to have such excellence in physical therapy in our town!

Mark Jamantoc did a fantastic job working on my knee and ankle post-op. I had a large rock fall on and break both of my legs. Recovery was long and tough but after only 3 weeks with Mark I was able to return to work as a mountain guide almost entirely pain free.

Performance Physical Therapy provided services at the US Amateur Soccer Association national championships, the Adult Soccer Fest. I don't think I could have played in the finals without their great care. They provided fantastic service for all the athletes. Throughout the week their staff were professional, concerned and friendly. The treatment of injuries was first class! Working on muscle pulls, taping, ice, caring for wounds, dealing with head injuries, etc - they were excellent.
They provided by far the best service we have ever experienced at this tournament, which has been held for 20 years

A wonderful team, from the reception to the therapists and assistants. Mark and Alicia helped get my shoulder back to 100% after a level 3 separation in a mountain bike mishap. They were great about evaluating me, starting with a progression, and then ramping it up as I made progress. At the end, Mark also helped me realize that I was holding back in my mind, and my body was ready to get back to normal.
Highly recommended.
