Rehabilitation Partners is a team of healthcare providers that serves as guardians of our patients' healthcare, fitness, and well-being. We care for patients in skilled nursing facilities and long-term care settings. And we provide house calls to homebound patients. Our patients are often discharged from acute-care hospitals after experiencing medical or surgical issues.
Many patients do not need extended hospitalization but are too unstable to be discharged home. We lead our community in delivering healthcare to our patients in their homes, which could be their homes or assisted and independent living facilities. With a shared passion for providing excellent care, the physicians, advanced care practitioners and support staff at Rehabilitation Partners are dedicated to having and treating patients, as well as one another, with compassion. We care for patients who are in-patient in skilled nursing, long-term care, assisted living and independent living facilities.
Wellness program including: personal fitness, tips on self-care, meditation classes, MBSR (mindful-based stress reduction) and a nurturing environment for a family like community. By being consistent, transparent and open minded, we have created a culture that allows us to retain and build team members, patients, and community affiliates.

We have several community resources and partners to help service a variety of your medical and non-medical needs. New in-home or housecall patients should fax or email the new patient packet to us for evaluation.

Our providers maintain and, if necessary, modify the care plans of the hospital while the patients receive skilled nursing and work with physical, occupational, and speech therapists.
We are a private medical practice started in 2011 by our current owners, Carrie Irivn, PA-C and Mark Gould, PA-C. Carrie and Mark worked together for another practice for several years. Their employer, Dr Bill McConnell suddenly and tragically passed-away in August of 2011 leaving Carrie and Mark to make some difficult choices.

I was in need of a Primary Care physician and was able to get in quickly to see Angela Baumeister. She was thorough, kind and sensitive to my immediate needs and long term health strategies. She made referrals for routine exams and to a specialist that would address my immediate concerns. Everyone in the practice has been wonderful to deal with.
