We provide service coordination for children, adolescents, and adults. Our Case Management is designed to assist in decreasing the odds to individuals whom may otherwise be homeless, incarcerated or institutionalized. Helping with medical managment, education, mental health, housing, employment and substance abuse.

Services
WMC provides an array of services to both adolescents and adults. Our mission to provide housing, adaptive living skills, outpatient services, substance abuse services, supported employment services, and other necessary supports to youth and young adults whom otherwise would be homeless, incarcerated or institutionalized.
This service is provided by a team of 3 people. It can consists of mental health and substance abuse services as well as other supports necessary to assist the client in achieving rehabilitation and recovery goals. It helps individuals gaining access to necessary services: reduce psychiatric and addiction symptoms; and develop optimal community living skills.
It would be our privileged for you to allow us your FIRST STOP WITH ANY OF YOUR CASE MANAGEMENT NEEDS, ASSISTING WITH RESIDENTIAL, SUPPORTED JOB, OUTPATIENT, MEDICATION MANAGEMENT, COMMUNITY BASED PROGRAMS, MENTAL HEALTH AND SUBSTANCE ABUSE SERVICES. Children, adolescents and adults will benefit from a variety of programs provided by our Westbury Medical Center Support Team.
