Services
We offer Comprehensive Clinical Assessments, Medications Management, Person Centered Treatment Plans for Clients & Partnering Agencies as well as Pastoral Care. If you are a new client, please complete the following Client Registration and Consents. You may either bring them or send them to us by fax or email prior to your first appointment. Additional items to bring include a valid id, insurance card, proof of guardianship for minors such as birth certificate and a list of medications if applicable.
Payments may be made in the form of cash, check, healthcare payment card, credit or debit card and are due at the beginning of each visit. Fees vary based on the type of service offered along with the degree and license type of the therapist. Please call the office for specific amounts. We do not wish to turn anyone away for inability to pay and at the therapist's discretion we offer a sliding scale fee based on income.
