Shalom Health Care Center has been awarded Patient-Centered Medical Home recognition by the National Committee for Quality Assurance PCMH Program. A Medical Home is all about you. Caring about you is the most important job of your Patient Centered Medical Home. In this personal model of health care, your primary provider leads the team of health care professionals that collectively take responsibility for your care.
When you come to Shalom as a patient, your first step is to meet with a financial counselor who will help explain the costs of your health care and help you figure out how to cover those costs. We'll help you find out if you're eligible for services at reduced cost or at no cost to you, through programs like Medicaid, Shalom Advantage, and the Prescription Assistance Program.
We are one of the few FQHCs in Indianapolis that receives funding from the Marion County Ryan White Program. We provide routine testing to all; targeted outreach for Ryan White who fall under the Minority AIDS Initiative. In the past three years, Shalom has screened over 5,000 clients (approximately 1,710 per year; which over 1,300 have been Ryan White eligible.)

Everything we do is designed to provide quality, professional primary care, behavioral health services, and wellness education to our community members. Shalom's School-Based Program provided medical care to students in 26 School-Based clinics, at no charge to the families!

I transferring doctors offices ASAP I am pregnant and was told tht I was going to received my baby’s gender a month ago thn on December 30 they had me come in for labs and said 10 days after tht I they would call with results they never called I called them a total for 3 times the first time they said they hadn’t got to it yet the second time said they got it but they could reach any medical assistance and now I called again and they are saying they have the results but they can’t tell

This clinic is a joke I have been a patient in the Suboxone program for almost 2 years.the Dr I had got a different job and they have let me run out of meds every week for the last 6 weeks in a row . This really seems like a race issue.Im sorry I am not Hispanic or black.but I'm sure if I were I would not be running out of my meds .I didn't even want to put the 1star but I had to

This review is only based on my experience here of getting a COVID vaccine.
They are located right off 56th street and Lafayette and was the closed location to get my booster shot. I had an appointment at 8:10, and the check in process was smooth and efficient.
There were several people waiting too so I still waited about 30 minutes there to be called.
So if you have an appointment here you still should expect to wait.
Once I got called it was very quick. This building was very clean and staff were professional.

This health care center is terrible!!!!! They don’t call you with your results or anything, you have to call them. Then they don’t send your medication on time!!! You have to call them about that also. I personally don’t mind calling them back, but if you tell me we are going to call you in 2 business days and it becomes 4 business day it’s a problem. It’s very unprofessional!!

It’s impossible to get through to them. After on hold for almost 30 minutes you’ll get hung up on only to have to call back and wait another 30 min. It’s happened every single time. The staff that checks you in are all super nice & sweet. However, the doctors seem rushed and never address your concerns.
