Services
Payment is due at the time of service. An estimated co-pay will be explained once eligibility is checked. Final patient responsibility may vary depending on how your insurance processes the claim. Any remaining balance will be due at that time. NOTE: If you are scheduled after hours or are scheduled in the American Fork or Springville offices, please use the ELAVON Patient Portal button to make a payment online.

Help stabilize persons in need by meeting their basic critical needs such as food, clothing, housing, transportation assistance, furniture and utilities, while providing them with the long-term solutions needed to rebuild their financial and social self sufficiency, enabling them to break out of poverty.Builds bunk beds for the needy, and delivers them assembled with mattresses and bedding, and installs them in the recipient's home/apartment.
